EXECUTIVE SUMMARY
Recommendation to accept an award of $104,475 in grant funds from the State of Florida
Department of Health, Bureau of Emergency Medical Services, which requires a local match of
$34,825, for the purchase of ten hydraulic assist stretchers and to approve necessary Budget
Amendments.
OBJECTIVE: To purchase hydraulic assist stretchers to improve the safety of patients and
field personnel.
CONSIDERATIONS: On April 14, 2015, the Board of County Commissioners approved the
submittal of a grant application to the State of Florida Department of Health, Bureau of
Emergency Medical Services for hydraulic assist stretchers in the amount of $417,900 in grant
funds and a required local match of $104,475. By letter dated June 1, 2015, we were awarded
$104,475 with a required $34,825 match. EMS has been in the process of field testing power
stretchers and will bring back recommendations for Board approval.
The purpose of the grant is to improve and expand Emergency Medical Services (EMS) by
assisting in the purchase of 10 hydraulic assist stretchers. There is no formal agreement for the
acceptance of this grant. The previously signed application affirms compliance with the terms
and conditions of the grant.
FISCAL IMPACT: Budget amendments are necessary to recognize $104,475 from the State of
Florida Department of Health, Bureau of Emergency Medical Services within EMS Grant Fund
(493) Project 33430 as well as appropriate the required 25% match in the amount of $34,825
within EMS Match Fund (494) Project 33430. Local funding for the match requirements is
available by transfer from the EMS Operating Fund (490).
LEGAL CONSIDERATIONS: This item has been reviewed by the County Attorney, is
approved as to form and legality, and requires majority vote for approval. -JAK
GROWTH IMPACT: There is no Growth Management Impact resulting from this action.
RECOMMENDATION: That the Board of County Commissioners accepts the grant proceeds
from the State of Florida Department of Health, Bureau of Emergency Services and approves the
Budget Amendments to recognize and appropriate the funds from the State of Florida
Department of Health, Bureau of Emergency Services and to provide the match from EMS Fund
490.

I am pleased to award Emergency Medical Services (EMS) Matching Grant, ID Code M4066 in the
amount of $104,475.00 to Collier County EMS Department'. Your required local cash match is
$34,825.00, with a total budget of $139,300.00. The purpose of this grant is to improve and expand
EMS by assisting your organization in the purchase of 10 hydraulic assist stretchers. Section
401.113(2) (b), Florida Statutes, authorizes and requires this grant program (number 64.003 in the
Florida Catalog of State Financial Assistance), which is funded through the Florida Department of
Health's EMS Trust Fund. There are no federal funds involved.
Your funds for the state amount will be sent in full, in advance, within approximately 30 days. The grant
begins the date of this letter and ends June 30, 2016. Please note it is a requirement that you report
grant activities and purchases to the state by the third week of November 2015, March 2016, and July
2016 (which will be your final report). Your signed grant application affirms you have read, understand
and will comply with the conditions and requirements in the "Florida EMS Matching Grant Program
Application Packet, December 2008.° You can obtain a copy from your identified contact person.
