EXECUTIVE SUMMARY
Recommendation to approve and authorize the Chairman to execute the attached Resolution
authorizing execution of Joint Participation Agreement Contract No. G0576 with the Florida
Department of Transportation to fund a fuel truck at the Immokalee Regional Airport in the
amount of$144,000.
OBJECTIVE: That the Board of County Commissioners (BCC), acting as the Airport Authority,
approve and authorize its Chairman to execute the attached Resolution authorizing execution of
Joint Participation Agreement Contract No. G0576 with the Florida Department of Transportation
(FDOT)to fund a fuel truck at the Immokalee Regional Airport(IMM) in the amount of$144,000.
CONSIDERATIONS: The Airport Authority has received funding approval from FDOT under
the Joint Airport Capital Improvement Program (JACIP) in the amount of$144,000 to fund a fuel
truck at IMM.
The Immokalee Regional Airport serves a broad range of customers that utilize Jet-A fuel. In
addition to transient jet traffic, the Airport has a based aerial applicator and jet turbine testing
facility that utilize approximately 150,000 gallons of Jet-A fuel annually. The jet turbine testing
facility has a ten-year lease. It requires the use of a mobile refueler for each test, and utilizes the
Airport's refueler for its fuel delivery needs.
IMM currently utilizes one of two Jet-A refuelers normally stationed at the Marco Island
Executive Airport to accommodate customers requiring mobile jet refueling capabilities. Both of
these refueler units are over twelve (12) years old, are constantly in need of costly repairs and have
been recommended for replacement by the Fleet Management Division.
With the expansion of Immokalee, Ave Maria and Eastern Collier County, IMM anticipates an
increase in the number of turbo-prop and jet aircraft that require mobile jet refueling capabilities.
These funds will enable the Airport Authority to purchase a new Jet-A refueler truck to provide
more efficient and reliable service with this industry standard mobile refueling amenity.
The total estimated cost to replace the fuel truck is $180,000. The FDOT JPA award is for
$144,000. The local match of$36,000 is available in Fund 497.

FISCAL IMPACT: Budget Amendments are necessary to recognize FDOT grant revenue in
project number 33451-01 in the amount of $144,000 within Airport Grant Fund 498, and local
match in the amount of$36,000 within Grant Fund 499. Budget Amendments are also needed to
reclassify local match funds in the amount of $36,000 currently in the motor pool replacement
project 57495 within Airport Fund 497.

GROWTH MANAGEMENT IMPACT: There is no growth management impact associated
with this Executive Summary.
LEGAL CONSIDERATIONS: This item is approved for form and legality and requires a
majority vote for Board approval. —JAB
RECOMMENDATION: That the Board of County Commissioners, acting as the Airport
Authority, approve and authorize its Chairman to execute the attached Resolution authorizing the

RESOLUTION OF THE COLLIER COUNTY AIRPORT AUTHORITY
APPROVING AND AUTHORIZING THE EXECUTION OF THE ATTACHED
JOINT PARTICIPATION AGREEMENT G0576 WITH THE FLORIDA
DEPARTMENT OF TRANSPORTATION
WHEREAS, the Collier County Airport Authority has the authority to enter into the
attached agreement with the Florida Department of Transportation to undertake projects as
authorized by Chapter 332, Florida Statutes, and Section 14-60 of the Florida Administrative
Code, and Ordinance 2004-03.
NOW, THEREFORE, BE IT RESOLVED BY THE Collier County Airport Authority,
Collier County, Florida, that:
1. The Joint Participation Agreement (JPA) Number G0576 to fund a Fuel Truck at
the Immokalee Regional Airport, Financial Management Number 433631-1-94-01,
attached hereto, is hereby approved.
2, The Chairman of the Collier County Airport Authority,Tim Nance, is authorized
to enter into, modify or terminate JPA Number G0576 with the Florida
Department of Transportation unless and until it may be specifically rescinded.
3. This Resolution shall take effect immediately upon adoption.
ADOPTED this 10th day of November 2015 after motion, second and majority vote in
favor of adoption.

Dear T & M Potolet Services:
This license authorizes, T & M Potolet Services to transport sludge in the vehicle
referenced above for the period from through . Transport and the condition
of the vehicle shall be in compliance with the requirements of Ordinance 87-79.
Your attention is directed to the following requirements of Collier County Ordinance
87-79:
1. It shall be unlawful for any person to transport sludge without first having
obtained a license from the Director unless exempt under Section 5 of this
Ordinance.
2. The above listed vehicle and sludge hauling containers will be maintained so
they remain product tight.
3. The above listed vehicle must display licensee's name, license number and
telephone number in at least three inch high letters and numbers.
4. The applicant shall give prior written notice to the Director of a change in
sludge disposal sites.

5. All licensees will maintain records of the sludge transported on a daily basis
and submit to the Director, a monthly summary of sludge transported. These
records will be due monthly by the 15th of each month for the proceeding
month's transportation activity and include the following:
A. Date
B. Name and location of generator (only for sewage
treatment plant sludge).
C. Type of sludge (i.e., sewage treatment plant sludge, septage,
grease, portable toilet waste, etc.).
D. Quantity of sludge pumped.
E. Disposal site.
Upon receiving this license, the designated license number TL06-16 must be placed on
your vehicle.
This license does not preclude the right or authority of any other County, State or
Federal agency from requiring separate licenses in accordance with rules and
regulations of that agency.
