EXECUTIVE SUMMARY
Request that the Board of County Commissioners approve the use of $15,000 from the
Confiscated Trust Funds to support the State of Florida Prescription Drug Monitoring
Program.
OBJECTIVE: To request that the Board approve the use of $15,000 to support the State of
Florida Drug Monitoring Program.
CONSIDERATION: The Florida PDMP Foundation, Inc. by virtue of Section 932.7055 (5)(c) 1
Florida Statutes is seeking Board authorization for the appropriation of funds for support of the
State of Florida Prescription Drug Monitoring Program. The Florida PDMP Foundation, Inc. has
applied to the Sheriff for appropriation. The CCSO requests Board approval to appropriate
$15,000 to support the Florida PDMP Foundation.
Some of the purposes of the State of Florida Prescription Drug Monitoring Program are:
a. Reduce overdose deaths and addiction rates in Florida.
b. Provides relief to millions of patients receiving legally prescribed pain medication.
The CCSO requests $15,000 from the Confiscated Trust Funds to be used to support their
purposes. This support will benefit Florida PDMP Foundation mission.
FISCAL IMPACT: Funds are available within the Confiscated Trust Fund to support this
appropriation
GROWTH MANAGEMENT: None.
RECOMMENDATION: That the Board of County Commissioners approves the appropriation of
$15,000 Confiscated Trust Funds for support the State of Florida Drug Monitoring Program.

On behalf of the Florida Prescription Drug Mon&rLg Program Foundation I would Ifke b bank your office for its past support
to keep the E-FORCSE database operational to help fight prescciplion drug abuse and diversion.Since it went online in 2011
the database has coed over 115M presaipiion records with about 26,000 PracIlloners utilizing it in addition,nearly 900
law enforcement agenda are authorized users having made over 43,000 requests for Information on active Irwesdgatlons.
The Florida legislature's approval of the database and new rules for opening pain managsmar tdinics has gay reduced
the pr am of uruegdated'P11 Mils'In the stale which has also led to a major dedere of about 41 percent in Florida
residents dying from controlled substance overdoses.
in approving the establishment of One E-FORCSE database,the leglsiehse elected to have the program funded by private
donations,but not from pharmaceutical industry.Thus,It established the POMP Foundation,a 501 c 3 tart-exempt private non-
profit it corporaion.The legislature also authorized the foundation to be a Direct Support Organization operating under the
Florida Department of Health.Its 11 member board of directors Is appointed by the State Surgeon General.
Since Ns inception,the foundation has sought funds from corporations,practitioners,local city and corny governments,health
care and law enforcement organizations.The DOH was also able to secure private,state and federal grant funds and a one-
time$500,000 allocation torn the legislature to get the program operational and maintain it through 2014.To ensure Its
congaed sustairabllty to provide patient,errand prescriber prescription g infomnabon to health care

enforcement users the POMP Foundation must continue to seek funding of about$600,000 per year.In June.
General Pam Bondi made a major gill to the foundation of$1.973M to ensure the basic operations of the E-FORCSE
database through 2018.However,(ands are slip needed for updating and enhancing the database's technology,to educate
practitioner;ioners and law enforcement about the information available from E-FORCSE and to keep the foundation operational so
it can seek the additional funds needed to nun the database properly.
Florida sheriffs have been major contributors to the success of the slate's Prescription Drug Monitoring Program.Since 2011,
the Florida Sheriffs Association and county sheriff offices have provided$319,500. The majority of sheriffs'donations have
come from Asset Forfeiture Funds. Collar Coudy sheriffs oboe has contributed$20,000.To obtain a list of all foundation

Betty Ford Foundation.
CONSIDERATION: The Hazelden Betty Ford Foundation by virtue of Section 932.7055 (5)(c) 1
Florida Statutes is seeking Board authorization for the appropriation of funds for support of the
alcohol and drug addiction treatment. The Hazelden Betty Ford Foundation has applied to the
Sheriff for appropriation. The CCSO requests Board approval to appropriate $5,000 to support
the Hazelden Betty Ford Foundation.
Some of the purposes of the Hazelden Betty Ford Foundation are:
- Inpatient, outpatient, and post-treatment addiction programs for adults and youth
- Patients receive individualize programming determined by a team of professionals
- Medication assisted help for prescription drug dependence
The CCSO requests $5,000 from the Confiscated Trust Funds to be used to support their
purposes. This support will benefit the Hazelden Betty Ford Foundation mission.
FISCAL IMPACT: Funds are available within the Confiscated Trust Fund to support this
appropriation.

Dear Kevin,
As the holiday season approaches,I'm writing to ask for your help. You can make sure no
child has a holiday season like the one Kristi had the year she saved the lives of her mother,
brother and sister.She was just 15.
A TERRIFYING CROSS-COUNTRY DRIVE
That year,Kristi's family spent Christmas in Lexington,Ky.Her mom,Amy,planned to drive
home to Forest Lake,Mimi with the children,while her husband,a pastor,traveled elsewhere
for work.
They weren't on the road long before Kristi realized something was very wrong.Her mom
seemed seriously sick;she was swerving dangerously all over the road.Kristi's younger siblings
were in the back seat,and she knew she had to do something.
Kristi had earned her learner's permit about a month earlier."I wasn't a very good driver,
but I said,`Mom,you have to let me drive.
"I went into survival mode.I knew we would die if my mom kept driving.I had to become
the mom for a little while."
Thanks to Kristi's courage,they made the 15-hour drive home safely with Kristi at the wheel
and Amy passed out in the seat next to her.A month or two later,Amy went to Hazelden at
Center City for an assessment and was admitted to treatment for alcohol addiction.
Today,Amy has been sober seven years.Thanks to contributions from
generous donors like you,she and her family were rescued from a life-threatening
struggle with alcoholism.
As the Director of Clinical Services at the Women's Recovery Center in Center City,I have
heard so many stories about women like Amy who hide their addictions,who can't bring
themselves to get the help they desperately need to save themselves and their families.
THE PERFECT WIFE AND MOTHER
Amy was a model mom for years before she began drinking.She was a busy pastor's wife,
church music director,mother of three beautiful children and stalwart member of the parish.
She kept dozens of balls up in the air without breaking a sweat.
