I. CALL TO ORDER
Chairman Fogg called the meeting to order at 10:00 A.M.
A quorum was established.

III. APPROVAL OF AGENDA
Ms. Fanning moved to approve the Agenda of the Forest Lakes Advisory Committee subject to
the following change:
Add: VII.A. New Business: Nominations of Officers for the Forest Lakes Advisory Committee
Second by Ms. Bell. Carried unanimously S- 0.
IV. APPROVAL OF MINUTES—November 7, 2017
Mr. Barry moved to approve the minutes of the November 7, 2017 Forest Lakes Advisory
Committee as presented. Second by Ms. Bell. Carried unanimously S- 0.
V. CONTRACTORS REPORTS
A. Windham Studio—Scott Windham (not present), no report this month.
A Purchase Order has been created and a Notice To Proceed (NTP) will be issued for Windham
Studios. Mr. Windham will prepare an assessment of plant material damage and submit a
summary report to Mr. Schumacher. The following items will be addressed:
• Storm damage assessment of trees and plantings.
• Compilation of recommended tree replacements balancing color preferences and storm
durability.
During Committee discussion the following was noted:

• Mr. Windham will present his tree inventory recommendations to the Committee at the
January 2018 meeting of the Forest Lakes Advisory Committee meeting.
• Mr. Gross, Ground Zero Landscaping, and Mr. Schumacher will accompany
Mr. Windham on the community drive through in preparation for the summary report.
• The Committee will act on the recommendations of Mr. Windham's summary report at
the February 2018 meeting.
B. Ground Zero Landscape Maintenance—Aaron Gross
Mr. Gross reported on landscape maintenance as follows:
• FEMA contractors removed a large percentage of storm debris on Woodshire Lane.
• Fertilizer will be applied by week ending December 9, 2017.
• The irrigation schedule was increased in two zones at the front entrance.
• Ground Zero Landscaping was awarded the Swale Maintenance contract to control weeds
and aquatic growth in the lakes.
• Grooming of the swales has been performed several times.
VI. PROJECT MANAGERS REPORT
A. Budget Report
Mr. Schumacher presented the"Forest Lakes MSTU Fund 159, December 5, 2017" for FY18
noting:
• FY18 Ad Valorem property tax revenue budget is $256,100.00 of which$1,413.75 has
been collected.
• Total revenue FY18 is $488,800.00 including investment interest,transfers and
contributions (minus a 5%reserve of$12,900).
• Operating Expense Budget is $162,100.00 of which$54,707.23 is committed; $8829.14
is expended.
• Unencumbered Operating Expense funds available are $98,563.63.
• A Purchase Order in the amount of$6,300.00 has been issued to Mettauer Environmental
for aquatic vegetation maintenance in the Quail Run Golf Club lakes. Service is provided
monthly. One treatment has been applied, with additional monthly treatments to follow.
• A Purchase Order in the amount of$11,500.00 has been issued to Preferred Material for
asphalt milling of a sixty foot lane width area of pavement at the Forest Lakes Boulevard
front entrance.
• A Notice To Proceed has been issued to Ground Zero for Swales maintenance within
Quail Run Golf Club and the corresponding PO should be listed in the January Budget
Report.
• A Purchase Order has been issued to Hart's Electrical Inc. for repairs to the electrical
mast on the meter box which powers the#2 and#3 fountains in Lake 14.
• The outstanding balance for Hannula Landscaping, a carry-over from the FY17 swale
maintenance contract, will be cancelled.
• The stormwater"Drainage Improvements"project will go out to bid in the first quarter of
2018 with construction scheduled in mid-April, as requested by the Quail Run Golf Club.
Funds are allocated in the FY18 budget.
• Total available FY-18 balance is $424,820.42.
During Committee discussion the following was noted:

• Mr. Schumacher will confer with the budget department to confirm availability of
sufficient funding for the"Drainage Improvement"project noting Reserves for Future
Construction, Line 40 in the budget, can be accessed for the work if necessary.
• Mr. Schumacher will contact Florida Power& Light to reconnect the meter which
powers the fountain for the Emerald Greens Condo Association Lake.
B. Hurricane Irma—September 10th.
Mr. Schumacher reported on recovery efforts:
a. Woodshire clean-up status and activities—
i. Reduced Scope
• FEMA contractors removed extensive debris material. Removal cost will be
borne by FEMA.
ii. Revised Quotes Requested
• Mr. Schumacher and Chairman Fogg met with county contractors to re-
evaluate the scope of work and secure proposals for remaining hurricane clean up
on Woodshire Lane.
• The M.S.T.U., as the contracting agency, will review the revised quotes and
award the contract.
• Mr. Schumacher contacted the management company for Naples Bath and
Tennis to request a financial contribution to the clean-up effort but a return call
has not been received.
C. Quote and Bid Solicitations
Mr. Schumacher reported as follows:
a. Quote(s)Requested: McShea/Roadway Thermoplastic,Municipal Sign/Street &
Stop Sign Replacement.
Roadway Striping
Mr. Schumacher distributed quotes from McShea Contracting, a county qualified
contractor, for striping perimeter roadways (Woodshire Lane and Forest Lakes
Boulevard)with Thermoplastic and installing reflectors.
During Committee discussion the following was noted:
• A combination of a center yellow line and white side lines is preferred.
• Forest Lakes Drive, Quail Forest Boulevard and the entrance to the community on
Forest Lakes Boulevard should be included in the project.
• Golf cart crossings, crosswalks, cul-de-sacs and stop bars will be incorporated.
• The landscaping contractor will edge the roadways.
Mr. Schumacher will request Traffic Operations install a white line for the entry turn to
Forest Lakes from Pine Ridge Road as well as a"Do Not Enter" sign on the exit side.
Chairman Fogg motioned to approve McShea Contracting install Thermoplastic center
and side line striping and reflectors on the Forest Lakes roadways of Woodshire Lane,
Forest Lakes Boulevard, Forest Lakes Drive and Quail Forest Boulevard at a cost not
to exceed$65,000.00. Stop bars, crosswalks, cul-de-sacs and golf cart crossings will be
included Second by Ms. Fanning. Carried unanimously 5—0.

Sign Replacement
Mr. Schumacher reported the stop and street signs damaged by Hurricane Irma will be
replaced in compliance with County's standards (the Manual on Uniform Traffic Control
Devices).
During Committee Discussion the following was noted:
• The left turn sign from Pine Ridge Road onto Woodshire Lane is down and the
MSTU historically has maintained the sign.
• Propose "no turn"verbiage for trucks in excess of a predetermined weight limit.
• Placement of a barrier in roadway to deter traffic is under the jurisdiction of
Traffic Operations.
Mr. Schumacher will:
• Include repair/replacement of the left turn sign from Pine Ridge Road onto
Woodshire Lane to the sign list.
• Refer the Committee's request for traffic deterrents to the County Traffic
Operations Department.
b. PO's Requested: Preferred Materials/Pavement Repair.
A Purchase Order in the amount of$11,220.00 and Notice To Proceed(NTP) have been
issued to Preferred Materials for asphalt milling to repair a pot hole and sixty foot lane
width area of pavement at the Forest Lakes Boulevard front entrance.
c. PO's Issued: Mettauer/Lakes Weed Control,Windham Studio/Landscape
Architecture, Ground Zero/Swales Maintenance.
Mr. Schumacher reported, as noted in the project manager's budget report, Purchase
Orders have been issued for the contractors listed.
d. Landscape Maintenance(December 31,2017).
Mr. Schumacher:
• Distributed an Invitation to Bid((ITB) for Landscape Maintenance services to
qualified contractors. Bids are due December 14, 2017. A one year replacement
contract will be executed by the end of December 2017.
• Reported swale maintenance, a stand-alone contract has been awarded to Ground
Zero Landscaping.
• Stated he is awaiting a response from Hart's Electrical for a price to conduct
monthly lamp inspections.
e. Light Pole Number Duplicates.
Mr. Schumacher will further evaluate re-numbering the thirty-one poles in the northeast
segment of Forest Lakes Boulevard.
Mr. Schumacher will investigate the cost of repair/replacement of the damaged &
downed light poles.

VII. NEW BUSINESS
A. Nomination of Officers for the Forest Lakes Advisory Committee.
Chairman Fogg solicited Committee Members to volunteer for officer positions on the Forest
Lakes Advisory Committee for FY-18.
Mr. Schumacher will research the date as specified in the By Laws for nomination and election
of officers to the Committee.

IX. PUBLIC COMMENTS
Name Tags
At the suggestion of Robert Monico, 1065 Forest Lakes Drive, Mr. Schumacher will provide
name tags for the Committee members.
Drainage and Swale Maintenance
Mr. Monico queried if in conjunction with renovation of the swimming pool, drainage water could
be pumped away from Phase I Villas. Mr. Schumacher noted the pool contractor should be
consulted to provide plans for drainage.
The M.S.T.U. is only involved with maintenance of the Golf course swales and ensuring they are
clear of debris weeds and aquatic vegetation. Homeowners are responsible for protecting their
property from water intrusion.
The county is conducting a"Gordon River Improvements Master Plan Scope of Services. " The
Scope of Services is developing a master plan for stormwater improvements for the Gordon River
Extension(GRE) in Collier County. The plan, which will include development of initiatives
necessary to improve the flood protection levels of service for areas within the watershed such as
Forest Lakes, may be beneficial to Forest Lakes' drainage problem.
Sidewalks
A previous investigation of the feasibility of sidewalks along Forest Lakes Drive determined space
limitations do not permit installation.
Roadway Speed Limits
With the exception of a segment of Woodshire Lane, roadways within the Forest Lakes community
are privately owned,thus limiting the jurisdiction of Collier County Traffic Ops and Sheriff's
Office to mandate or enforce a 25 or 30 MPH speed limit. There is no Master Homeowners
Association and multiple Associations would have to concur and sign off on a speed limit policy.
The Advisory Committee discussed the applicable regulations in February 2017 and concluded not
to enforce a community-wide policy.
