I. Call to Order
Chairman Fogg called the meeting to order at 10:00 A.M.
A quorum was established.
II. Attendance

III. Approval of Agenda
Ms. Fanning moved to approve the Agenda of the Forest Lakes Advisory Committee as presented.
Second by Mr. Goody. Carried unanimously 4- 0.
IV. Approval of Minutes—August 1,2017
Chairman Fogg moved to approve the minutes of the June 6, 2017 Forest Lakes Advisory
Committee meeting as presented. Second by Ms. Fanning. Carried unanimously 4- 0.
V. Contractor Reports
A. Windham Studio—Scott Windham (not present) Report Attached.
The monthly community landscape report dated August 29, 2017 noted the following items:
Entry West ROW
• Cut the rain sensor PVC riser to within 12" above the controller box. Action is complete.
Main Entry Island
• Cassias have been removed; replacement Bougainvillea standards are installed.
Forest Lakes Blvd.
• Trim limbs out of head clearance zone above sidewalk on trees noted in report at Light
Pole #'s 94, 99, 116, 121, 129, 156, 159, 172, 176, 163 and 167.
• Light Pole #'s 106, 126, 140 and 145 —Cassia trees - Stand up, re-stake.
• Light Pole # 126— Solicit opinion from Scott Windham regarding recommendation to
remove suckers from the Cassia tree.
• Light Pole #135 —Golden Shower tree to be straightened and re-staked.
• Light Pole #148—Monitor health of largely defoliated pink Tabebuia. Minimal new
foliar growth observed.

MINUTES
• Light Pole#155—Recommend replacement of Floss Silk tree. Seek Scott Windham's
opinion regarding decline in health of Floss Silk tree and ask him to recommend
replacement species.
Mr. Schumacher will discuss the Floss Silk tree condition with Mr. Windham and report his
findings to the Committee.
Woodshire Lane
• Trim Brazilian Pepper growth off Light Pole head#'s 22, 39 and 41.
• Trim Hong Kong Orchid limbs off of sidewalk and lamp head at Light Pole#'s 29 and
33.
• Light Pole #'s 48—50: Clean Potato vine off of perimeter fence.
• Three Laurel Oaks, Light Poles#50& 51, are in ill health; removal recommended.
• During Committee discussion the following was noted:
o The Cassia Trees should be removed and replaced with 5'—6' Bougainvillea
during the rainy season.
Mr. Schumacher will request Naples Bath and Tennis remove the Brazilian Pepper from the
fence. Trimming will be requested if the invasive exotic is not removed.
Tree Removal at Fairways at Emerald Greens ("Fairways")
• If the declining Laurel Oak trees at the Fairways Condominium are deemed a"safety
hazard" by a certified Arborist,then they must be removed.
• Mr. Schumacher has researched the MSTU easement in the vicinity of the trees, obtained
for the drainage, sidewalk, lighting and landscaping installed by the MSTU.
• The County Attorney's Office (CAO) advised the condo association it is responsible for
the landscaping not installed by the MSTU.
• Mr. Schumacher notified the property manager for the Fairways, Southwest Property
Management, of their responsibility to evaluate the trees and remove if necessary.
Mr. Schumacher will contact the Fairways again to secure their response to tree removal
noting Code Enforcement will be notified if the Association fails to take action.
B. Ground Zero Landscape Maintenance—Aaron Gross
• Mr. Gross will schedule service on items detailed in Windham Studios report.
VI. Project Managers Report
A. Budget Report
Mr. Schumacher presented the "Forest Lakes MSTU Fund 159, September 5, 2017"for FY17
noting:
• $182,758 of the $189,600 FY 17 Ad Valorem Tax budget has been collected.
• The Operating Expense budget is $167,383 of which$33,078 is committed and $117,232 in
invoices has been paid.
• Operating Expense available balance for FY17 is $17,072.83.
• Line 38 `Reserves' will fund the planned Drainage Structures project, designed by ABB.
• The re-striping of roadways project will be funded in the FY-18 budget.
• Unused funds can be transferred between purchase orders to accommodate shortages if
necessary.
• Total Available Balance is $250,515.

B. August 25,2017 Rain Event
Mr. Schumacher introduced Dale Fey, Fire Marshal,North Collier Fire Control and Restrict
District. Mr. Fey reported he was attending the meeting to address questions and concerns
regarding community flooding.
Chairman Fogg distributed a 2009—August 2017 annual and historic rainfall event recap:
• Average historic rainfall for the community has been 54 inches.
• Rainfall for 2017 as of August 31, 2017 was 61.1 inches.
• The storm on August 27, 2017 produced 8.45 inches of water.
During Committee discussion the following was noted:
• Pumps are not available to lower the water level in the lakes.
• Water drainage from the community onto other property is not permitted.
• The Collier County Stormwater Division is conducting a study to determine opportunities
to lower water tables.
• The Fire Department does not provide portable radios during storm events for residents.
• The Fire Department does not provide service when storm conditions have achieved a
certain level.
• Boats and large vehicles are available for evacuation.
• Communities are encouraged to establish a Community Emergency Response Team
(CERT).
C. Quote and Bid Solicitations
Mr. Schumacher reported as follows:
a. Lakes Weed Control Maintenance. (N/A)
• Weed control maintenance service has not been performed in two months.
• Quote(s) for this service will be requested.
• The MSTU plans to assume maintenance responsibility on or after October 2017 (FY18).
b. Golf Course Swales Maintenance. (September 30,2017).
• Quotes (due September 22, 2017) for a six month service contract have been requested
from three qualified contractors.
c. Landscape Maintenance. (December 31,2017)
• Ground Zero Landscaping's contract expires in December 2017.
• Mr. Schumacher is preparing an Invitation to Bid((ITB) for Landscape Maintenance
services from qualified contractors.
• Staff has drafted a comprehensive contract bundling landscaping, swale maintenance and
a lighting report.
• Bids will be reviewed at a November or December 2017 Advisory Committee meeting.
D. Tree Replacements
a. Golden Shower Ornamental at Light Pole 138.
• The Golden Shower tree has been replaced.
b. Cassia Trees with (15) Bougainvillea's at Forest Lakes Blvd entrance.
• The specified 25 gallon trees were purchased at the 15 gallon price and installation is
complete.

E. Sidewalk Lighting—Lamp Outages
Lamp outages have been replaced.
F. Light Pole Number Duplicates
a. Forest Lakes Blvd—Re-number 31 Poles in the Northeast Segment
Mr. Schumacher distributed a graphic illustration of the Phase II lighting for Forest Lakes
Boulevard and a proposal to re-number thirty-one duplicate light poles in the northeast
segment.
Mr. Schumacher will obtain a quote and issue a notice-to-proceed for Ground Zero
Landscaping to re-number duplicate poles 63 to 92.
G. Quotes
No proposals were presented.

A. Roadway Re-striping & Reflectors
Mr. Schumacher contacted and is awaiting a response from "Better Roads, Inc. "for the
following:
a. Paint or Thermoplastic
Life expectancy of paint versus thermoplastic road re-striping.
b. Pavement install date and estimated Asphalt service life
The date of previous asphalt installation of the roads in Forest Lakes.
Mr. Schumacher noted the MSTU is only responsible for the main roads in the development
and recommended discussion of roadway re-striping be placed on the October 2017 Agenda.
The work will be allocated to the FY-18 budget.
B. Golf Course Sand
Mr. Burnham informed Mr. Schumacher the golf course sand stored in the vicinity of
Woodshire Lane and Forest Lakes Boulevard will be re-located by the end of September.
C. Water Accumulation at the Intersection of Forest Lakes Boulevard and Woodshire Lane
The Committee addressed the water accumulation occurring on the sidewalk at the intersection
of Forest Lakes Boulevard and Woodshire Lane.
During Committee discussion the following was noted:
• Maintenance of the area is under the MSTU's jurisdiction and the condition is a safety
hazard.
• Methods to prevent water accumulation including installation of a perforated drainage
pipe to direct water underneath the sidewalk to a catch basin, should be investigated.
• An engineering firm should be retained to design a solution.
Mr. Schumacher will investigate drainage options to alleviate water accumulation.

MSTU Plan for Hurricane Irma Recovery
1. Evaluate situation as soon as safely possible. First week (Sept 11-17)
• Remove MSTU landscape that is blocking roadway. Stack debris in Med
• Remove MSTU landscape that is blocking sidewalks. Stack on existing
landscape
• Remove hazardous conditions, leaning trees etc.
• Inspect equipment for damage/safety issues
• Make report of damage
2. Evaluate remaining landscape with landscaper. Week 2-3 (Sept 18-Oct 1)
• Designate landscape for Immediate removal and schedule.
• Designate Landscape for Prune and observe and schedule
• Remove Debris and schedule
• Activate irrigation system and test completely- Make repairs
• Repair, damaged equipment (Broken sidewalks, fencing, lighting, etc.) and
schedule.
• Perform all of Item 2.
3 Evaluate remaining landscape- Week 4-24 (Oct 2-Feb 26 2018)
• Replace destroyed plants as desired.
• Replace plants that did not recover as desired.
• Perform normal weekly maintenance.
4. File claim for damage as soon as actual costs are known.
