I. Call to Order
Chairman Fogg called the meeting to order at 10:02 A.M.
A quorum was established.
II. Attendance

Ms. Fanning moved to approve the Agenda of the Forest Lakes Advisory Committee subject to the

IV. Approval of Minutes—June 6, 2017
Mr. Goody moved to approve the minutes of the June 6, 2017 Forest Lakes Advisory Committee
meeting as presented. Second by Ms. Bell. Carried unanimously 4- 0.
V. Contractor Reports
A.Windham Studio—Scott Windham (not present) Report Attached.
The monthly community landscape report dated July 25, 2017 noted the following items:
Main Entry Island
• Sod has filled in well with summer rains; no patching required.
• Cassia trees continue to decline as a result of a leaf fungus generic to the species.
A proposal to replace the Cassia's with Bougainvillea Standards has been submitted by
Ground Zero Landscaping.
Forest Lakes Blvd
• Trim Mahogany limbs off light pole # 93 head.
• Trim limbs out of head clearance zone above sidewalk at Light Pole#'s 102, 116 120 and
129.
• Light Pole #148—Monitor health of largely defoliated pink Tabebuia. Minimal new foliar
growth observed.
• Light Pole #132—Remove and replace Golden Shower tree. Ground Zero Landscaping
has provided a proposal. Mr. Schumacher noted this is a maintenance item.

Woodshire Lane
• Trim Brazilian Pepper growth off Light Pole#41 and off of sidewalk from over fence
Light Pole#44
• Trim Hong Kong Orchid limbs off of sidewalk at Light Pole#46.
• Three Laurel Oaks, Light Poles #50 & 51, are in ill health; removal recommended.
During Committee discussion the following was noted:
• The Cassia Trees should be removed and replaced with 5' —6' Bougainvillea Standards
during the rainy season.
Tree Removal at Fairways at Emerald Greens
• If the declining Laurel Oak trees at the Emerald Greens Condominium are deemed a
"safety hazard"by a certified Arborist, then they must be removed.
• Mr. Schumacher has researched the MSTU easement in the vicinity of the trees, obtained
for the drainage, sidewalk, lighting and landscaping installed by the MSTU.
• The County Attorney's Office (CAO) is reviewing the easement and will offer an opinion
on whether the condo association is responsible for the landscaping not installed by the
MSTU.
• Upon the CAO response, Mr. Schumacher will notify the Fairways at Emerald Greens of
whose responsibility it is to have the trees evaluated and possibly removed.
Ms. Fanning motioned to proceed with removal of three Laurel Oak Trees on Woodshire Lane
at the Fairways of Emerald Greens Condominium Association if it is determined to be an MSTU
responsibility by the County Attorney's Office. Second by Ms. Bell. Carried unanimously 4—0.
Light Pole Numbering System
Mr. Schumacher will secure a proposal to re-number light poles with duplicate numbers.
B. Ground Zero Landscape Maintenance—Aaron Gross
Mr. Gross reported maintenance is proceeding as scheduled.
VI. Project Managers Report
A. Budget Report
Mr. Schumacher presented the"Forest Lakes MSTU Fund 159, August 2, 2017"for FY17
noting:
• $182,699 of the $189,600 FY 17 Ad Valorem Tax budget has been collected.
• The Operating Expense budget is $167,383 of which$33,608 is committed and $110,855
in invoices has been paid.
• Operating Expense available balance for FY17 is $22,920.47.
• Line 24 `Lighting Maintenance' was increased$3K to fund Hart's Electrical replacement
of lamp outages and will be reflected in the September budget.
• Line 38 `Reserves' will fund the planned Drainage Structures project, designed by ABB.
• Hannula Landscaping's schedule for swale maintenance was modified to weekly service in
June 2017.
• The re-striping of roadways project will be funded in the FY-18 budget.
• Unused funds can be transferred between purchase orders to accommodate shortages if
necessary.
• Total Available Balance is $256,362.

During Committee discussion the following was noted:
• Ground Zero Landscaping's contract expires in December 2017.
• Mr. Schumacher is preparing an Invitation to Bid((ITB) for Landscape Maintenance
services from qualified contractors.
• The contract will bundle landscaping and swale maintenance.
B. Lakes Weed Control Maintenance—Purchasing Process Underway.
Mr. Schumacher reported Staff is preparing an Invitation to Bid to be issued to qualified vendors
for weed control maintenance for the lakes.
During Committee discussion the following was noted:
• Contractors must be qualified to bid on the contract.
• Cost effectiveness of a bundled contract combining landscape and swale maintenance vs. a
free standing contract for swale maintenance will be evaluated.
• Mr. Schumacher will verify the perimeter distance for quote purposes.
• Chairman Fogg recommended the "littoral zone"(the area between low and high water
marks)be considered for measurement. He will email suggested verbiage to Mr.
Schumacher.
C. Sidewalk Lighting—Lamp Outages
Mr. Schumacher reported on lamp outages as follows:
• Forest Lakes Boulevard—approximately twelve lamp outages. Hart's Electrical will
replace the bulbs.
• Lamps out from the southwest back strip of Forest Lakes Boulevard to Woodshire Lane
Pole#44. Hart's Electrical will determine the cause of outage and report their findings to
Mr. Schumacher.
• Lake 14: The LED fountain light is not functioning. Hart's Electrical will inspect the
control panel and photo cell and repair; a purchase order for maintenance is on file.
D. Roadway Re-Striping and Reflectors—
a. Paint
• McShea's standard paint contains glass beads and a reflective material and meets FDOT
standards.
• Life expectancy is six to eighteen months.
b. Thermo-plastic
• Life expectancy is six to seven years.
• Thermo-plastic can be applied over existing striping material.
During Committee discussion the following was noted:
• Thermo-plastic application for cross walks and golf cart crossings and paint striping for the
roads could be an option.
• Mr. Schumacher will research the date community roads are scheduled to be re-paved.
Mr. Schumacher recommended roadway re-striping be placed on the October 2017 Agenda and
bids be solicited to be allocated from the FY-18 budget.

E. Quotes
Mr. Schumacher presented a quote from Ground Zero Landscaping for Committee consideration
to replace thirteen Cassia trees with twenty-five Bougainvillea Standards at the main entrance of
Forest Lakes Boulevard.
The Committee reviewed the proposal and a photo of the specimen provided by Mr. Gross.
Chairman Fogg motioned to approve the proposal from Ground Zero Landscaping,Inc. number
E375, dated July 25, 2017 in the of$4,947.50 to remove thirteen Cassia trees from the Forest
Lakes Boulevard circle median and replace them with twenty-five Bougainvillea Standards.
Second by Mr. Goody. Carried unanimously 4- 0.
F. Tree on Woodshire Lane
Mr. Schumacher received a notice from the Road Maintenance Department that a fallen Royal
Poinciana tree had been removed from Woodshire Lane. The MSTU cleaned the area of
remaining debris.
VII. New Business
A. Sand on Golf Course not Screened
Golf Course sand stored in the vicinity of Woodshire Lane and Forest Lakes Boulevard is visible
to some residences of Emerald Greens.
The Committee noted the sand should be re-located or enclosed by an opaque structure.
Mr. Schumacher will contact Mr. Burnham to discuss the sand storage and ascertain Club plans
to remediate the problem.

IX. Public Comment
Lee Dixon, Turtle Lakes Golf Colony Condominium Association commended the Committee on
drainage improvements and swale maintenance. He noted water accumulation occurs on the sidewalk
at the intersection of Forest Lakes Boulevard and Woodshire Lane and requested the Committee
explore a solution to address the issue.
During Committee discussion the following was noted:
• Maintenance of the area is under the MSTU's jurisdiction and the condition is a safety hazard.
• Methods to prevent water accumulation including installation of a perforated drainage pipe to
direct water underneath the sidewalk to a catch basin, should be investigated.
Mr. Schumacher will investigate drainage options to alleviate water accumulation.
Chairman Fogg noted some cul-de-sacs and properties on west Forest Lakes Boulevard accumulate
water due to inadequate swale drainage. Although the drainage in this vicinity is not an MSTU's
responsibility, the water does overflow onto MSTU's roadways.
Mr. Schumacher will review the area and determine possible solutions to the flooding.

• Cut the rain sensor PVC riser to within 12" above the controller box so it is
not so top heavy and make sure it is securely screwed into threaded base.

trunks with no outer bark which are also in ill health. We recommend having
the trees removed, but also recommend that they be evaluated for potential
removal by a certified arborist for the final decision. (FIRST TREE STILL IN
JEOPARDY)
GENERAL:
• The Cassia Tree decline is mostly attributed to the leaf fungus that has
become the norm for the species. The only management tool is a two-prong
approach of a foliar fungicide (Alliete) and a systemic fungicide (Subdue).
Ground Zero has treated the Cassia Trees with this method. If the MSTU
Committee elects to remove and replace the Cassia Trees at a future time,
we recommend the following options:
• SMALL SCALE TREES: Seminole Pink Hibiscus or other
color (6'ht)Year-round flowers, Bougainvillea (Sundown
Orange, New River Purple, or Barbara Karst) (5' ht) Fall /
Winter Flowers, Plumeria - Bridal Bouquet (6' Ht) Summer
flowers
• MEDIUM SCALE TREES: Muscogee Crepe Myrtle (10' ht)
Summer flowers, Dwarf Poinciana (7-8'ht) Summer flowers
• We recommend flush cutting the Cassias and planting the
replacement trees 3 feet further back from the curb in the
same spacing.
• County will coordinate re-numbering the light poles along the northeast
section of Forest Lakes Blvd to complete the sequence without duplication.
