Call to Order Chairman Fogg called the meeting to order at 10:00 A.M. A quorum was established. II. Windham Studio—Scott Windham (not present)Report Attached. Mulch. Mr. Schumacher presented the "Forest Lakes Monthly Maintenance Report" dated February 28, 2017 from Scott Windham, Landscape Architect, Windham Studio, Inc. based on community drive through with Mr. Windham and Mr. Gross and highlighted the following: • Mulch ➢ Mr. Windham recommends re-mulching for soil moisture retention and weed control as well as aesthetics. • Cassia Trees ➢ The Cassia Tree decline is mostly attributed to the leaf fungus that has become the norm for the species. Recommend treating in the spring(April/May)with a foliar fungicide (Alliete) and a systemic fungicide (Subdue). • Main Entry Island ➢ Weed infestation in the St. Augustine grass in the entry island has been treated with Atrazine. ➢ Bare areas will be patched with sod in the spring. • Forest Lakes Boulevard ➢ Pink Tabebuia at light pole #167 is largely defoliated and health should be monitored. ➢ Trim one remaining dead Mahogany tree limb at light pole#93 above sidewalk for vertical clearance. ➢ Monitor Orange Geiger at light pole#150 for foliar flush. ➢ Clean cobwebs from lamp at light pole #161. • Woodshire Lane ➢ Two Laurel Oaks at light poles#26—28 should be evaluated for potential removal by a certified arborist; Mr. Windham noted the trees could become a hazard. B. Ground Zero Landscape Maintenance—Aaron Gross Mr. Gross reported he has addressed the following landscape items: • Mulch application is complete. • The Cassia Trees have been treated with a fungicide. • The defoliated Laurel Oak trees on Woodshire Lane have been tagged with a yellow ribbon. • Cob webs have been brushed off the street light lens at light pole#161. 2 • A quote in the amount of$146 was presented for the removal of two dead Cassia trees at the main entrance—Flush Cut to ground level. During Committee discussion the following was noted: • Two Cassia trees in the circle median should be removed. Chairman Fogg requested Staff recommend a plan of action for the Cassia trees. Mr. Schumacher will consult with Mr. Windham and Mr. Gross and make a recommendation including estimated cost proposal at the April 2017 meeting. • Maintenance of the Laurel Oak trees on Woodshire Lane, not planted by the MSTU, is the responsibility of the Fairways at Emerald Greens condominium association. Chairman Fogg motioned to approve the proposal from Ground Zero Landscaping,Inc. number E353, dated February 27, 2017 in the of$146.00 to remove two Cassia trees from the circle median. Second by Ms. Belk Carried unanimously 5-0. Chairman Fogg moved to inform Fairways at Emerald Greens that the Laurel Oak trees on the fence side of Woodshire Lane are in poor condition,possibly posing a safety hazard, and the Forest Lakes M.S.T.U. recommends the trees be removed Second by Ms. Belk Carried unanimously 5-0. C. Agnoli,Barber& Brundage(ABB) 1. Presentation: Outfall Study Reports Jim Carr and Kellie Fissinger of ABB presented the PowerPoint"Forest Lakes MSTU Drainage Improvements North and West Outfall Study"dated January 26, 2017 noting four options were studied and an opinion of probable construction cost and conclusions drawn as follows: • Gravity Pipe to West Swale - $135,303 o Water flows in opposite direction • Force Main to West Swale - $189,996 o Floods west basin • Force Main to Pine Ridge- $143,942 o Very minimal improvement to intersection water level • Force Main to West Dissipater- $330,984 o Very minimal improvement to intersection water level Mr. Carr did not recommend implementation of any of the four scenarios modeled. Mr. Carr distributed the "Gordon River Improvements Master Plan Scope of Services"to the Committee. The Scope of Services requests ABB develop a master plan for stormwater improvements for the Gordon River Extension(GRE) in Collier County. The plan will include development of initiatives necessary to improve the flood protection levels of service for areas within the watershed such as Forest Lakes. ABB is anticipating receipt of a Purchase Order from the County and Mr. Carr emphasized the project may be beneficial to Forest Lakes' drainage problem. Mr. Burnham revealed the Quail Run Golf Club is discussing reconstructing the golf course and the course drainage system,potentially alleviating some of the flooding in Forest Lakes, and requested communication be kept open. 3 The Committee directed Staff to initiate construction of the project detailed below: Infall 1. Catch Basin&Piping Enlargement. Proceed with Tasks A, B and C of the current Design project, PO 4500168571, as documented in Exhibit A (previously distributed to the Committee). a. Replacing structure JB5 on the Southside of Forest Lake Boulevard. b. Replacing an existing 24" storm drain pipe with 36"pipe between Forest Lakes Boulevard and Lake 18. c. Installing an 18" drain to the north side of Forest Lakes Boulevard into JB5. The "Opinion of Cost" submitted by Agnoli, Barber and Brundage totaled $93,737 for the stormwater improvements. $12,000 allocated for Roadway and Drainage Allowance may not be required and$16,150 budgeted for concrete endwalls may be reduced if the bank and rip rap can be restored. VI. Project Managers Report A. Budget Report Mr. Schumacher presented the "Forest Lakes MSTU Fund 159, March 7, 2017" for FY17 noting: • $163,955 of the $189,600 FY 17 Ad Valorem Tax budget has been collected. • The Operating Expense budget is $167,383 of which$93,915 is committed and $48,564 in invoices has been paid. • Operating Expense available balance for FY17 is $24,905. • Agnoli, Barber& Brundage engineering will be billing for the "Outfall Study." • Total available balance of$309,288 plus funds committed and not spent yields Estimated Cash balance of$403,203. Mr. Schumacher distributed a recap of the amortization schedule for Fund 259, Forest Lakes Debt Service,for informational purpose noting the bond is on schedule to be paid off in 2022 and any shortfall in funds will be covered by reserves. B. Quail Run Easements—New Board of Directors Elected to Quail Run GC Mr. Schumacher reported the Quail Run Easements for the Forest Lakes Roadway and Drainage M.S.T.U. for the maintenance program to control weeds and aquatic growth in the lakes is ready for delivery to the Quail Run Golf Club Board of Directors. Mr. Burnham informed the Committee the Quail Run Golf Club is electing new officers. Mr. Schumacher requested Mr. Burnham notify him on the names of the new Board members so the easement package can be distributed to the Directors. 4 C. Fence Line Maintenance Chairman Fogg reported on project construction noting: • The next phase will be to landscape the space between Woodshire Lane and the shopping area. • Work will continue related to the project. D. Property Tax Assessments Mr. Schumacher apprised the Committee of the County's procedure Ad Valorem Property Tax valuations and assessments as follows: • Preliminary Ad Valorem Tax Valuations for FY17 will be assigned this coming July. • FY17 TRIM(Truth in Millage)Notices will be distributed to property owners in August 2017. • Property Taxable Values will be finalized and certified in October 2017. • The percent variance+/- is listed on the current MSTU budget summary (FY16 to projected FY17 adjusted by+8.53%). • The Forest Lakes MSTU assesses millage at a fixed 4.0 rate,with a portion of the millage allocated to bond payments and the balance apportioned to operating expenses. • Upon repayment of the outstanding bond(projected to be in 2022),the MSTU may consider establishing a"revenue neutral"budget, meaning that MSTU millage would then be adjusted annually to maintain revenues at a predetermined target level. E. Lake 14 Fountain#3 Hart's Electric determined that the fountain controls are operating correctly, indicating a problem with the pump. Mr. Schumacher issued a purchase order to Florida Fountains and Equipment (FFE) to evaluate the pump. VII. New Business A. Roadway Markings - Striping& Reflectors requested by Emerald Greens Condo Association Mr. Schumacher distributed a copy of an email received from Ruth Prior,resident of the Fairways at Emerald Greens, on February 7, 2017 requesting the MSTU replace the roadway reflectors on Woodshire Lane. Mr. Schumacher has requested quotes from McShea Contracting outlining options for roadway markings and will report his findings at the April 2017 meeting for Committee review. During Committee discussion the following was noted: • The recently painted striping on the Golf Cart Paths and Pedestrian Crosswalks is fading. Mr. Schumacher will confer with McShea Contracting to ascertain the reason for the "dingy" appearance of the striping and report to the Committee. VIII. Old Business A. MiraVista Mr. Schumacher reported Ms. Sillery is continuing to research for information regarding potential former construction of the area. 5 IX. Committee Member Comment None X.
