Call to Order Chairman Fogg called the meeting to order at 10:02 A.M. A quorum was established. II. Attendance Members: George Fogg, Chair; Marianne Fanning, Vice Chair; Richard Barry; Barbara Bell; John Goody County: Dan Schumacher, Project Manager; Harry Sells, Project Manager Other: Aaron Gross, Ground Zero Landscaping; Doug Burnham, Quail Run Golf Club; Wendy Warren, JuriSolutions III. Approval of Agenda Mr. Barry moved to approve the Agenda of the Forest Lakes Advisory Committee meeting subject to the following change: Addition: Item III. Approval of Agenda—A. Pledge of Allegiance Second by Ms. Bell. Carried unanimously 5- 0. A. Pledge of Allegiance The Committee discussed the Board of County Commissioners request to recite the Pledge of Allegiance at the beginning of County meetings, noting the decision is at the Committee's discretion. Mr. Goody motioned to approve the Board of County Commissioners request to recite the Pledge of Allegiance at the Forest Lakes MSTUAdvisory Committee meetings. Without a second the motion was not considered. IV. Approval of Minutes—January 3,2017 Chairman Fogg moved to approve the minutes of the January 3, 2017 Forest Lakes Advisory Committee meeting as presented. Second by Ms. Fanning. Carried unanimously 5- 0. V. Contractor's Report A. Windham Studio—Scott Windham (not present)Report Attached. Mulch. Mr. Schumacher presented the"Forest Lakes Monthly Maintenance Report"dated January 31, 2017 from Scott Windham, Landscape Architect, Windham Studio, Inc. highlighting: 1 • Mulch ➢ Mr. Windham recommends re-mulching for soil moisture retention and weed control as well as aesthetics. • Directory Sign Area ➢ Orange Geiger Trees have been treated with herbicide—continue to monitor for Geiger Beetles. • Main Entry Island ➢ Weed infestation in the St. Augustine lawn entry medians and directory sign area have been treated with herbicide. ➢ Recommend sod installation in"bare" areas. • Forest Lakes Boulevard ➢ Trim Shady Lady limbs at light pole#167 to ten foot sidewalk clearance. Mr. Gross noted trimming trees more than ten feet in height is not in his "Scope of Work. "as defined in their contract. The trimming is performed by Ground Zero on a time and material basis. ➢ Trim Mahogany tree limbs at light pole #93 above sidewalk for vertical clearance. ➢ Monitor health of pink Tabebuia at light pole#148. • Woodshire Lane ➢ Two Laurel Oaks should be evaluated for potential removal by a certified arborist. Mr. Gross has marked the two Laurel Oak trees in jeopardy with yellow flags for ease of identification by the Committee. ➢ Clear Brazilian Pepper encroaching on the sidewalk at light poles#'s 19, 26, 27 and 28. B. Ground Zero Landscape Maintenance—Aaron Gross Mr. Gross reported he had addressed the following landscape items: • Fertilizer application is complete. • Turf weeds at front entrance have been sprayed with Altrazine herbicide. • Spider webs have been brushed off street light lenses. • Tree pruning to reduce sight line issues and improve visibility has been done. • Quotes were presented for replacement of two dead Cassia trees at the main entrance. This item will be placed on the March Agenda for consideration. During Committee discussion the following was noted: • Evaluation of reason for Cassia tree decline. Mr. Gross noted "tree girdling"occurred as a result of staking reinforcements placed on the trees. • Green Island Ficus and Crown of Thorns pruning should be layered. • Light pole lenses should be cleaned regularly. VI. Project Managers Report A. Budget Report Mr. Schumacher presented the"Forest Lakes MSTU Fund 159, February 7, 2017"for FY17 noting: • $150,042 of the $189,600 FY 17 Ad Valorem Tax budget has been collected. • The Operating Expense budget is $167,383 of which $118,006 is committed and $24,424 in invoices has been paid. • Operating Expense available balance for FY17 is $24,953. 2 • Funds committed and reflected in the budget for Agnoli,Barber&Brundage engineering, including the current Outfall Study, are $27,371. o $12,565 Stormwater Engineering Services o $13,920 Study of North and West Stormwater Drainage Outfall • Final payment of Phase I—ABB Engineering Services has been approved. • A Purchase Order for and Notice-to-Proceed have been issued to Windham Studio, Inc. for FY-17. • Other Contractual, Line 17, includes funds for Hannula Landscaping Swale Maintenance and Ground Zero. The $1,121.46 recorded in "red"for Hannula Landscaping reflects a closed purchase order. • Bond debt service payments are not reflected in the Fund 159 Operating Budget. • Total available balance of$309,495 plus funds committed and not spent yields Estimated Cash balance of$427,501. Mr. Schumacher conferred with the County Budget Director and verified, subject to review and approval by the accounting office and the Board of County Commissioners,the $232,700 in reserve funds may be accessed for future projects. Mr. Schumacher will email the Committee the recap of the amortization schedule for Fund 259, Forest Lakes Debt,for informational purposes. B. Quail Run Easements Mr. Schumacher reported Ms. Arnold, Mr. Burnham and he met with the Quail Run Golf Club Board of Directors in January 2017 to discuss the easements and the prospect of the Forest Lakes Municipal Service Taxing Unit(MSTU)taking over the maintenance responsibilities of the lakes and ponds on the Quail Run Golf Club property highlighting the following: • The MSTU Advisory Committee previously motioned to accept the maintenance responsibility. • Easement documents must be executed for the purpose of enabling access to the subject lakes. Based on a letter dated February 7, 2017 from Ms. Arnold to Dr. Dennis Kelly, President, Board of Directors, Quail Run Golf Club,the following concerns were addressed at the meeting: • Clarification of the Subordination Statement in the easement. Ms.Arnold noted subordination applies to property encumbered by a mortgage. In the event the mortgage goes into default and the bank forecloses, the easement is subordinate to the banks interest. The easement can remain in effect or expire at the discretion of the mortgage holder. • Clarification of the Maintenance Agreement document with the 5 day cancellation provision. Investigation by staff revealed the Agreement was drafted by an earlier Quail Run Board and was not signed by the County. • The MSTU is committed to assuming maintenance consistent with the scope of services currently under contract privately by the club entailing weed control around the perimeter of each lake or pond, limited to the riprap installed by the MSTU. Upon execution and approval of the Easement documents, including approval by the Board of County Commissioners, Staff will solicit contractor bids in order to begin maintenance. Staff 3 further recommends a cancellation policy of this Lakes Maintenance Service Agreement may be initiated by either party upon a minimum of forty-five (45) days' notice. Mr. Schumacher presented a"Quail Run Golf Club Lake Maintenance Scope of Service" draft letter to the Advisory Committee and recommended a motion to proceed. Mr. Barry motioned for the Forest Lakes Roadway and Drainage M.S.T.U. to proceed with acceptance of the Quail Run Golf Club Lake Maintenance Scope of Service maintenance program to control weeds in the lakes and ponds within the perimeter of the Quail Run Golf Club. Request for Quotes (RFQ)from certified contractors will be solicited. Second by Mr. Goody. Carried unanimously S- 0. C. ABB Stormwater Engineering - Outfall Study Mr. Schumacher met with Jim Carr, Agnoli, Barber& Brundage on January 26. 2017and noted the following: • The North and West Outfall Study will be completed by the end of February 2017. • ABB will present results of the Study at the March 7, 2017 Advisory Committee meeting. D. Roadway Re-striping (Paint)— 1. Cart Crossing and Crosswalks scheduled for Tuesday 1/31 -overnight Mr. Schumacher reported McShea Contracting re-striped the Golf Cart Paths and Pedestrian Crosswalks. The project came in within the $5K budget allocated. E. Traffic Safety and Golf Cart Rules Mr. Schumacher distributed a list of applicable Florida statutes covering golf carts noting, with one roadway exception, Forest Lakes is mostly a private road community and there is not a Master Homeowners Association. The Committee discussed the regulations and agreed not to enforce a policy. F. Fence Line Maintenance—Work at the Crossings Center is underway by Green Effex. The Committee reported on project observations noting: • Damaged Ficus bordering Woodshire Lane and the shopping center is removed to the fence. • Plants including the root system should be completely removed. • The irrigation system is not functioning. Code Enforcement is monitoring the plant removal by Green Effex. Mr. Schumacher will follow up to ensure work is progressing as expected and will seek to determine what contractor will provide replacement plants. G. Property Tax Assessments Mr. Schumacher apprised the Committee of the County's procedure Ad Valorem Property Tax valuations and assessments as follows: • Preliminary Ad Valorem Tax Valuations for FY17 will be assigned this coming July. • FY17 TRIM (Truth in Millage)Notices will be distributed to property owners in August 2017. 4 • Property Taxable Values will be finalized and certified in October 2017. • The percent variance+/- is listed on the current MSTU budget summary (FY16 to projected FY17 adjusted by+8.53%). • The Forest Lakes MSTU assesses millage at a fixed 4.0 rate,with a portion of the millage allocated to bond payments and the balance apportioned to operating expenses. • Upon repayment of the outstanding bond(projected to be in 2022),the MSTU may consider establishing a"revenue neutral" budget, meaning that MSTU millage would then be adjusted annually to maintain revenues at a predetermined target level. VII. New Business A. Pine Ridge Road Pavement Detachment—Road Maintenance Division Notified Mr. Schumacher contacted the Road Maintenance Division to request repair of the damaged asphalt on the east bound lane adjacent to the sidewalk(west of the Forest Lake entrance). This repair is not the responsibility of the MSTU. VIII. Old Business A. MiraVista Mr. Schumacher reported Ms. Sillery is continuing to research for information regarding potential former construction of the area and noted Jim Carr,ABB,will check if they have survey data for the location. IX. Committee Member Comment None X. Public Comment Mr. Burnham reported the lights at the entrance by the Quail Run sign are not functioning and queried regarding the responsible party for maintenance Mr. Sells advised Mr. Burnham the Quail Run Golf Club maintenance department is responsible the lights.
