I. Call to Order
Chairman Fogg called the meeting to order at 10:00 A.M.
A quorum was established.
II. Attendance
Members: George Fogg, Chair; Marianne Fanning, Vice Chair; Richard Barry;

III. Approval of Agenda
Chairman Fogg moved to approve the Agenda as presented. Second by Ms. Bell. Carried
unanimously S- 0.
IV. Approval of Minutes—December 6,2016
Ms. Fanning moved to approve the minutes of the December 6, 2016 Forest Lakes Advisory
Committee meeting as presented. Second by Mr. Barry. Carried unanimously S- 0.
V. Contractor's Report
A. Windham Studio—FY17 Proposal and Cost Review
Mr. Schumacher reported the December community drive-through and"Forest Lakes Monthly
Maintenance Report" was not submitted as the Landscape Architect contract is not currently in
effect,pending review and motion by the Advisory committee.
Mr. Schumacher presented the Windham Studio, Inc. proposal for "Forest Lakes M.S.T.U.
Maintenance Consulting Services for FY 2017 per the Fixed Term Professional Landscape
Architectural Services Contract" for Committee consideration.
During discussion the Committee recommended the contract be accepted for TASKS 1 and 2
(Field Review Meetings and Monthly Field Reports) and TASK 3 (Attend M.S.T.U. Meetings)be
on an"as requested"basis. TASK 4 (Reimbursable Expenses)will be billed on a time and
material basis.

Chairman Fogg motioned to approve the contract for "Forest Lakes M.S.T.U. Maintenance
Consulting Services for FY 2017 per the Fixed Term Professional Landscape Architectural
Services Contract#16-6598"dated December 1, 2016 for Windham Studio,Inc. in the amount
of$6,150.00 with the provision meeting attendance will be on an "as requested"basis. Second
by Ms. Fanning. Carried unanimously 5—0.
B. Ground Zero Landscape Maintenance—Aaron Gross
Chairman Fogg reported the community landscaping generally looks good.
Mr. Gross reported he had addressed the following landscape items:
• Geiger trees were sprayed for beetles.
• Queen Crape Myrtle trees were treated for sooty mold.
• Irrigation on the main entry island is functioning as intended.
• Spider webs will be brushed off street light lenses - Chairman Fogg suggested an evening
drive-through to identify street lamps requiring cleaning.
• Tree pruning to reduce sight line issues and improve visibility is not part of Ground Zero
Landscape's Maintenance"Scope of Work" as defined in their contract. The trimming is
performed by Ground Zero on a time and material basis.
Mr. Schumacher will evaluate the trees for encroachment on the street lights during the
monthly drive-through with Mr. Windham and Mr. Gross.
VI. Project Managers Report
A. Budget Report
Mr. Schumacher presented the"Forest Lakes MSTU Fund 159, January 3, 2017" for FY17
noting:
• $110,654 of the$189,600 FY 17 Ad Valorem Tax budget has been collected.
• The Operating Expense budget is $167,383 of which$116,427 is committed and$16,097
in invoices has been paid.
• Operating Expense available balance for FY17 is $34,858.
• Funds committed for Agnoli, Barber and Brundage engineering activities:
o $12,565 Stormwater Drainage Structures Design
o $13,920 Stormwater Drainage Outfall Study
• Reserve Funds total $232,700.
• Total available balance of$320,690 plus funds committed and not spent yields Estimated
Cash balance of$437,117.
Mr. Schumacher noted, subject to review and approval by the accounting office, reserve funds
may be available for future construction projects such as the Agnoli, Barber and Brundage
stormwater drainage structures.
Chairman Fogg requested an update on the schedule for future Ad Valorem Property Tax
valuations and assessments.
Mr. Schumacher will research when in the calendar year the 2018 property assessment valuations
are set and report his findings to the Committee.
• A Purchase Order for fertilizer will be issued week ending January 7, 2017.
• Mulch will be applied post fertilization- Chairman Fogg queried areas to be mulched and
requested Mr. Windham's opinion on the necessity of application be solicited.

Mr. Schumacher will obtain information and report to the Committee.
• Hannula Landscaping swale maintenance is performed bi-weekly and Chairman Fogg
inquired whether this interval may be increased to every third week during the dry season.
Mr. Schumacher will investigate with Quail Run staff and report to the Committee.
B. ABB Drainage Projects
Mr. Schumacher reported on the proposed drainage improvements for the Forest Lakes MSTU by
Agnoli, Barber and Brundage (ABB).
1. Stormwater Drainage Structures Design-
a. PO End Date: Dec 09, 2016
b. The"Plan and Profile Schematic and Opinion of Cost"were presented to the Committee.
c. Deliverables have been received and are under review.
d. An invoice for this contract has been received and payment is pending.
2. Stormwater Drainage Outfall Study-
a. PO End Date: Feb 28, 2017
b. Work is underway on this two-task activity.
Upon discussion of the tabulated"Opinion of Cost"data, Mr. Schumacher recommended the
Committee await the results of the Outfall Study to consider the feasibility and potential cost
benefits of proceeding with both the `Structures' and `Outfall' projects simultaneously.
C. Traffic Study and Standardization
Mr. Schumacher reported Ms. Arnold recommended a study previously conducted be reviewed
prior to inviting a representative from the County Traffic Division to attend a meeting of the
Committee.
During Committee discussion the following was stated:
• The MSTU's scope of authority for standardizing signage and speed limits should be
clarified.
• Speed zones within a community must meet sufficient speed and be consistent for the
Sheriff's Office to enforce the limits.
• The public and private roadways within the Forest Lakes MSTU need to be identified.
Mr. Sells will print a document from the County's web site identifying public versus private
roads within the Forest Lakes MSTU and distribute it to the Committee for review.
Chairman Fogg motioned for the M.ST.U. Staff to draft a scope of work study to determine
M.S.T.U.parameters for establishing and enforcing traffic and signage regulations within the
Forest Lakes Community. Second by Mr. Goody. Carried unanimously S—0.
D. Perimeter Lamp Outages—Completed
Mr. Schumacher reported the replacement of non-functioning lamps is complete.
VII. New Business
A. Roadway Re-striping—Cart Crossing and Crosswalks Quote Requested
Mr. Schumacher reported he has received a quote from McShea Contracting within the $5K
approved budget to re-stripe the Golf Cart Paths and Pedestrian Crosswalks. A Purchase Order
will be requested the week of January 7, 2017 and the project will be scheduled with McShea.

B. Pine Ridge Road
The Committee inquired on the status for the repair of the damaged asphalt on the east bound lane
adjacent to the sidewalk(west of the Forest Lakes entrance).
Staff will contact the Road Maintenance division to ascertain if there are plans to repair the area.
VIII. Old Business
A. Fence Line Maintenance—Naples Bath & Tennis and Pine Ridge Crossing
• Pine Ridge Crossing: Mr. Schumacher met with representatives from Collier County
Code Enforcement and Kite (shopping center property management). Work will
commence once the irrigation pump is replaced. Mr. Schumacher will follow up with
Code Enforcement to ensure work is completed.
• Naples Bath&Tennis: Code Enforcement stated the plant material which Forest Lakes
MSTU Advisory Committee is requesting to be removed and/or pruned is "grandfathered
in" and consequently,they do not have jurisdiction over removal or pruning of the foliage.
Mr. Schumacher will provide the contact information to Code Enforcement for Chairman
Fogg and request they communicate with him on maintenance.
B. Quail Run Easements—Available Meeting Dates after January 15th
Mr.Burnham proposed a meeting be scheduled January 17,2017 with the Board of the Quail
Run Golf Club, Ms.Arnold,Ms. Zimmerman and Mr. Schumacher to discuss delegation of rip
rap maintenance. He suggested the meeting be held at the Quail Run Golf Club at 10 AM or
2 PM stating a preference for the 2 PM time.
Mr. Schumacher will confirm the date to Mr. Burnham by January 10, 2017.
C. Mira Vista
Mr. Schumacher reported Ms. Sillery had located a meeting notice pertaining to MiraVista from
2010. She will continue to research for information regarding the meeting and topics of
discussion.
IX. Committee Member Comment
Ms. Fanning queried on the policy for Golf Cart usage on the streets and noted some users do not
obey traffic rules and regulations.
Mr. Burnham will address the concern and review the policy for golf cart use with his Staff and
report his findings to the Committee at the next meeting.
