EXECUTIVE SUMMARY
Recommendation to approve Amendment No. 4 to the Agreement with the Department of Children
and Families for the Criminal Justice Mental Health Substance Abuse Grant, Amendment No. 3
with the Collier County Sheriff’s Office, and reinstate National Alliance on Mental Illness of Collier
County’s subagreement as well as approve the associated amended grant budget and application.
OBJECTIVE: To provide additional forensic intensive reintegration services to those persons involved
or are at risk of involvement with the justice system in Collier County.
CONSIDERATIONS: The Criminal Justice Mental Health Substance Abuse (CJMHSA) program was
established by the State to provide funding to counties. These funds can be used to plan, implement, or
expand initiatives that increase public safety. The fund can also be used to avert increased spending on
criminal justice, and improve the accessibility and effectiveness of treatment services for adults and
juveniles who have a mental illness, substance abuse disorder, or co-occurring mental health and
substance abuse disorders and who are in, or at risk of entering, the criminal or juvenile justice systems.
On June 10, 2014, the Board approved the County’s agreement with the Department of Children and
Families (DCF) and subsequent subrecipient agreements with David Lawrence Mental Health Center,
Collier County Sheriff’s Office, and National Alliance on Mental Illness (NAMI) of Collier County, Inc.
for the CJMHSA program (Agenda Item 16D9). The agreements became effective July 1, 2014 and will
expire June 30, 2017.
On September 9, 2014, the Board approved the First Amendment which added provisions for proper
accountability (Agenda item 16F1). On November 18, 2014, the Board approved the Second Amendment
to modify the subagreement with NAMI to reflect a staffing change and add State and Federal laws, rules,
and regulations (Agenda item 16D6).
On December 1, 2014, Community and Human Services (CHS) received notification from NAMI of
Collier County that they wanted to terminate their agreement.
On June 23, 2015, the Board approved the Third Amendment to the master agreement with Collier
County and the Department of Children and Families, which deleted NAMI as a subcontractor and the
Second Amendment to CCSO and DLC. (Agenda item 16D10).
On November 10, 2016, the Collier County Sheriff’s Office notified CHS that they were anticipating that
they would have an unexpended balance of $59,961 during the final year of the grant cycle. CHS
subsequently worked with DCF to reallocate the funds and make revisions to the master agreement and
subsequent CCSO subrecipient agreement. Accordingly, the budget was modified by reducing the
personnel line item by $33,715 and revising the FTE allocation. The remaining $26,246 will still be
allocated to CCSO. The following table is a summary of the re-allocated funds and the changes to the
allocated FTEs:

Applying the budget reductions, CHS reached out to NAMI to determine if the available funds could be
utilized. NAMI is a subrecipient in the upcoming CJMHSA grant 2017-2020, which was recently
awarded to Collier County. These funds will assist NAMI in hiring and initiating service before the
commencement of the new cycle. NAMI will hire a .8 FTE peer specialist to provide services such as
resource coordination, supported employment, motivational interviewing, supported housing, group
counseling, and assistance with the attainment of disability benefits.
In summary, the following changes have been made to CCSO’s subrecipient agreement in the third
amendment:
1. Extend the contract term
2. Reduce budget
3. Revise the FTE allocation
The master agreement between DCF and Collier County is also amended to reflect the following changes:
1. Add NAMI as a subcontractor
2. Increase budget for administration
The following changes were made to NAMI’s subrecipient agreement:
1. All references to Housing, Human and Veteran Services have been amended to reflect the current
name for the division: Community and Human Services
2. Exhibit A amended to remove paragraphs A1-7 and A1-8
3. Exhibit B-8.2 amended Contract Limits
4. Modify Exhibit C-2.1 Staffing to reflect staffing change
5. Add Exhibit C-2.3.7 to reflect required source documentation per DCF
6. Modify Exhibit C-2.4.1.1 to reflect the requirement of quarterly source documentation
7. Exhibit F-3 and F-7 have been modified to reflect the new budget in funds and match
8. Exhibit G and G-1 were replaced
9. Board directed Corrective Action language were added
10. Exhibit I, State and Federal Laws, Rules and Regulations have also been added as required per
DCF
11. Provisions for proper accountability over state resources and a property clause for the purchase of
tangible property
12. Extends the contract term
The application was amended at the request of DCF to reallocate money and services and reflects the
following changes:
1. Add NAMI as a subcontractor
2. Update Planning Council Members
3. Replace Corizon with Armor as a designated jail healthcare service provider

4. Update key personnel roles, level of effort, and cost
FISCAL IMPACT: The proposed changes will provide a net zero effect on the overall program budget.
The amount that was reduced from Collier County Sheriff’s Office will be allocated to NAMI and the
administration of the program by the Community and Human Services to utilize the resources more
efficiently. The funds will remain in Fund 707 Project 33332.
GROWTH MANAGEMENT IMPACT: There is no Growth Management Impact associated with this
Executive Summary.
LEGAL CONSIDERATIONS: This item is approved for form and legality and requires a majority vote
for Board approval. - JAB
RECOMMENDATION: That the Board of County Commissioners approve and authorize the Chairman
to sign the Fourth Amendment to the Agreement with the Department of Children and Families for the
Criminal Justice Mental Health Substance Abuse Grant, Third Amendment with the Collier County
Sheriff’s Office, reinstate NAMI of Collier County’s subagreement, and approve the amended grant
budget and application.
