meeting subject to the following changes:

V. Contractor's Report
A. Windham Studio—Scott Windham not present (report attached)
Mr. Schumacher presented the "Forest Lakes Monthly Maintenance Report" dated
August 31, 2016 from Scott Windham, Landscape Architect, Windham Studio, Inc. noting:
• Replace (3) dead Euphorbia plants on the south end of the median.
• Balance of items has been addressed with routine maintenance.

During Committee discussion the following was noted:
• The Ficus hedge at the Entry West ROW is not an MSTU responsibility and
recommendation for replacement should be removed from the report.
• Future reports should solely reflect new items for consideration.
Mr. Schumacher will inform Mr. Windham future reports are to contain current landscape
project recommendations relevant for Committee consideration.
B. Ground Zero Landscape Maintenance—Aaron Gross
Mr. Gross reported he addressed the items cited in Mr. Windham's status report noting:
• The (3) Euphorbia plants on the south end of the median were damaged by fallen Palm
branches and are now sprouting new growth. The plants will be monitored for continued
improvement.
• Four light pole locations will be addressed for encroaching branch pruning.
C. Agnoli,Barber & Brundage (ABB)Work Order: "Stormwater Engineering" Update
Mr. Brundage, P.E. President,Agnoli, Barber& Brundage, Inc. presented a Power Point
Presentation titled "Forest Lakes Stormwater Improvements 2016. "The report presented a
schematic overview of the Drainage System and alternative recommendations for water
management and drainage relief for the Forest Lakes Community highlighting:
• A 25 year storm model calibration was applied to the geographical representation of the
system.
• Three alternative improvement projects were outlined for consideration, one "infall" and
two "outfall" as follows:
Infall
1. Catch Basin& Piping Enlargement. Proceed with Tasks A, B and C of the current
Design project, PO 4500168571, as documented in Exhibit A(previously distributed
to the Committee).
a. Replacing structure JB5 on the southside of Forest Lake Boulevard.
b. Replacing an existing 24" storm drain pipe with 36"pipe between Forest
Lakes Boulevard and Lake 18.
c. Installing an 18" drain to the north side of Forest Lakes Boulevard into JB5.
Outfall
2. Gordon River Canal System Approach
a. Install a drainage system between the Forest Lakes Boulevard Entry to the
canal bordering the West side of the Community, transgressing between
Camelia Lane and Daisy Lane.
b. A drainage easement exists in this area.
c. Estimated construction cost is $313,000 including the previously approved
construction cost of$15K for Scheme 1.
3. Pine Ridge Drainage System Approach
a. Install a drainage system connecting Forest Lakes Boulevard Entry to the
existing Pine Ridge Road drainage system.
b. This scenario requires ABB to meet with two Collier County agencies and
Southwest Florida Water Management District for approval.

c. Estimated construction cost is $212,000, including the previously approved
construction cost of$15K for Scheme 1, and assumes utilities can be relocated
at a cost of$20K.
Mr. Brundage noted Approach 1 will offer improve drainage results and is an integral part of the
more comprehensive proposal.
The Committee discussed the proposals and will pursue consideration for Schemes 2 and 3 at a
future meeting.
The Committee requested Staffprovide a large scale map for reference during MSTU meetings.
VI. Project Managers Report
A. Budget Report
Mr. Schumacher presented the "Forest Lakes MSTU Fund 159, September 8, 2016"for
information purposes and provided an overview of the budget noting:
• Property taxes collected to date total $176,003 with a balance of$6,396 remaining.
• Of the $158,900 FY16 Operating Expense budget, $36,600 is committed and $103,800 in
invoices has been paid leaving $15,500 available for expenditures excluding reserves.
• Estimated cash available including committed and unspent funds totals $357,577.
• The FY 2017 budget is effective October 1, 2016.
• Reserve funds of$201,100 may be accessed to finance a major project such as Stormwater
Drainage with Board of County Commissioners approval.
• The millage rate is evaluated on an annual basis and although it can be adjusted it cannot
exceed the maximum rate set by the Board of County Commissioners.
• A portion of the millage rate is allocated to bond payments; the balance is appropriated to
operating expenses.
• The FY 2017 budget is effective October 1, 2016.
Ms.Arnold noted the primary goal of the Forest Lakes Roadway &Drainage MSTU is to improve
drainage in the community.
B. Update on Easements—Quail Run Golf Club
Mr. Schumacher presented a series of maps from the Collier County Property Appraiser's office
defining the boundaries of the MiraVista Condominium. The purpose of the report was to
determine the MSTU's responsibility for roadside swales, culvert maintenance, sidewalks and
lighting on Quail Forest Boulevard, specifically in the vicinity of the MiraVista Condominium.
• A search conducted of records of easements pertaining to the MiraVista Condominium
complex did not reveal any easements obtained from 1981 to present.
• An easement is required to perform work on private property.
• The MSTU does not have authorization and is not responsible to perform services for the
MiraVista Condominium Association at this time.
Ms.Arnold will research existence of easements pertaining to the Forest Lakes Community and
report her findings to the Committee at the October meeting.

VI. New Business
A. Crotons at the Forest Lakes Entrance—Go/No-Go (Ground Zero 6/15 quote)
Mr. Schumacher submitted a proposal from Ground Zero Landscaping in the amount of
$1,031.75 to plant Crotons at the Forest Lakes entrance.
The Committee discussed the proposal and agreed the current landscaping is attractive and
enhancement is not necessary.
Chairman. Fogg motioned to forego the installation of Crotons at the entrance to the Forest
Lakes Community. Second by Ms. Bell. Carried unanimously 4—0.
B. Ground Zero Contract Renewal
Mr. Schumacher reported the current contract for Ground Zero Landscaping is effective through
December, 2016 and requested the Committee consider a continuation of service.
Mr. Goody motioned to renew the contract with Ground Zero Landscaping for 2017. Second by
Ms. Bell. Motion carried unanimously 4—0.
C. MiraVista Condominium—Report on Drainage/Lighting/Sidewalks
Mr. Stemples, President MiraVista Condominium Association introduced photographs of recent
flooding in the swale on the MiraVista's Condominium property. During presentation he asserted:
• Historically the MiraVista property did not flood during heavy rains.
• MSTU Staff previously requested his signature on documents to upgrade the swale and
Mr. Stemples complied.
• Since construction, the restructured swale has flooded for extended periods of time and
overflow encroaches on the parking lot.
Mr. Stemples requested the MSTU address the issue to alleviate the flooding and recommends
drilling of two additional outflow holes in the weir, as the MSTU was responsible for creating the
problem.
During Committee discussion the following was noted:
• Ms.Arnold will investigate construction plans and the documents signed by Mr.
Stemple.
• Determination of responsibility will be assessed.
• Staff will research a solution to the drainage issue.
The topic will be placed on the October Agenda for discussion and recommendations.
VII. Old Business
A. Light Pole Cleaning—What Processed Used?
Mr. Goody reported Turtle Lakes Golf Colony Condominium Association maintenance staff
cleaned the street lights with a solution of baking soda and vinegar.
Staff and the Committee extended their appreciation for providing the service.
B. Thank you letter—Forest Lakes Boulevard and Wilshire Lane Vegetation Clearance
Mr. Schumacher queried if the Committee could provide the name of the individual to whom a
thank you letter should be addressed.

The Committee did not have knowledge of the appropriate party and suggested Mr. Schumacher
contact Mr. Burnham.
IX. Committee Member Comments
Ms. Bell requested Staff invite Jerry Kurtz, Stormwater and Environmental Planning, Collier County,
to attend one of their meetings to gain information on the Gorden River project and its potential
impact on the Forest Lakes drainage system project.
Ms. Bell reported a Eucalyptus tree was damaged and requested the tree be reinforced.
Mr. Gross, Ground Zero Landscaping, stated the tree was removed and a proposal for replacement
has been provided to Mr. Schumacher.
The item will be placed on the October meeting Agenda.
Ms. Bell requested a workshop to review the budget format.
Mr. Sells made himself available to work with Ms. Bell after the meeting.
X. Public Comment
Mr. Burnham updated the Committee on improvements to the Quail Run Golf Club noting $40,000
was spent on maintenance of the golf greens.

Forest Lakes MSTU Cul-de-sac and adjacent Property Owners,
In September 2012 the Forest Lakes Municipal Service Taxing Unit(MSTU)completed street tree
plantings side-walk construction and lighting improvements for Forest Lakes Boulevard adjacent to the
22 cul-de-sac streets within Forest Lakes Community. The MSTU hopes that you are enjoying these
enhancements to your community.
This letter is to clarify the maintenance responsibility for the landscaping that was installed as part of the
beautification project. The landscape portion of the project consisted of tree plantings and sod. The trees
are now past their initial period of establishment and the watering bags have been removed. The stakes
which were installed at planting have also been removed to allow for proper growth of the trees now that
their roots have been established.
The MSTU will be maintaining the street trees planted and each property owner will continue to mow the
sod around the tree and between the sidewalk and street. Please take care when mowing or weeding so as
not to damage the bark of the tree. The mulch ring, installed to retain water for the establishment of the
trees. will eventually deteriorate and be covered with your lawn. This is a desirable situation as the
design is for grass to grow all the way up to each tree trunk. The MSTU will perform all arborist related
services to the street trees and requests that you not attempt to maintain the trees near your property. Any
cutting of limbs will be the responsibility of the MSTU. The plantings in round-a-bouts in all of the 22
cul-de-sacs will continue to be maintained by the residents in each cul-de-sac.

The following is a brief maintenance status report for the Forest Lakes Common Areas:
DIRECTORY SIGN AREA:
• No action required.
CIRCLE MEDIAN:
• Replace (3) dead Euphorbia on north side of circle. (Ground Zero to provide
quote)
ENTRY EAST ROW:
• No action required.
ENTRY WEST ROW:
• No action required.
MAIN ENTRY ISLAND:
• Replace (1) dead Euphorbia on south end of median. (Ground Zero to
provide quote)
• In general, there are significant infestations of weeds in the St. Augustine
lawn in the entry medians and directory sign area. Strategize herbicide spray
and patch with sod. Then keep up with weeds throughout year. Ground Zero
will begin treating the weeds with Atrazine as soon as the weather cools
down.
• FUTURE: We recommend filling the space where the Oleander was with
either 7 gallon Ixora 'Trinidad', sourced from Boynton Botanicals or Petra
Croton to match circle median and directo sign.

• Light pole# 10- Trim Poinciana limbs off of sidewalk to 10' vertical
clearance.
• Light pole#113-(1) Yellow Elder was removed by an adjacent home owner
due to trunk snapping off at grade during a storm event. Ground Zero quoted
replacement with a White Geiger to match adjacent trees.
• Light pole# 116, 118—Trim Floss Silk limbs from light pole heads
• Light pole# 126- Trim Cassia limbs off of sidewalk.
• Light pole#135 - Trim Poincaina limbs off of sidewalk to 10' vertical
clearance.
• Light pole#148— Pink Tabebuia largely defoliated. Monitor health closely.
• Light poles 168-170-Trim Crepe Myrtle limbs off of sidewalk.
WOODSHIRE LANE:
• Light pole#51 - Laurel Oak is unhealthy with limited foliage and heavy
Spanish Moss. We recommend having the tree evaluated for potential
removal by a certified arborist.
• Light pole#46 - Trim Hong Kong Orchid limbs off walk
• (Golf course responsibility) Clean up debris, vining weeds and trim Brazilian
Pepper at maintenance facility fence.
• Light pole#36, 39, 41 —Contact Naples Bath and Tennis to trim Brazilian
Pepper limbs which are growing over the fence and around light pole
encroaching into the sidewalk.
