Refinement
1.Provide a detailed and specific explanation of the requested change(s)to the task(s)and/or the additional days
added:This change requires a design modification to the terminus of the project to the southbound lanes on Collier
Boulevard through the Green Boulevard intersection in order to allow a safer transition from three lanes to two lanes. Within
the current design, the lane transitons mimicked what had been done at other locations in the County. It is now known that
operationally this configuration presents problems for merging traffic within the area of the intersection and leads to more
accidents. An example of this configuration is at Airport-Pulling Road and Vanderbilt Beach Road intersection which the
County is now modifying under a separate project.
2. Provide detailed rationale for the requested change:As stated above, experience has borne out that lane transitons
within an intersection introduces more conflicts within the intersection and the potential for accidents. The rationale for the
change is implementing a safer and operationally functioning roadway system and meeting current design standards.
Modifying the current design will impact many of the construction drawings. This project has yet to be advanced to the
construction phase; therefore, it is prudent that this change is included in the current design of this project.
3. Provide explanation why change was not anticipated in original scope of work:The original scope of services was
created over 9 years ago and if it was known then what is known now, the original scope would have been written to
anticpate this change. The project has also gone through several design changes over the last nine years which included
changes to the project limits, the additon of a new bridge at Green Boulevard, several design modifications to meet current
standards and development impacts. All these changes created impacts that were not anticipated originally.
4. Describe the impact if this change is not processed: The project as designed will have to be constructed and then
another project will have to be created to make changes to a substandard roadway resulting in thousands of dollars of throw
away.

PART 1:WORK DIRECTIVE OVERVIEW
The CONSULTANT has been requested and retained to provide additional engineering services
associated with the final design and preparation of construction documents for Collier County
Project No. 68056, Collier Boulevard—Phase I (Green Boulevard to Golden Gate Boulevard).
These additional services are based on design changes and scope of work additions requested by
the Collier County Growth Management Division Transportation Engineering Department,
Roadway, Bridge & Stormwater Section required to modify the Collier Boulevard southbound
transition from the new 3-lane section to the existing 2-lane section. Collier County Traffic
Operations staff requested this modification to improve traffic movements/operations at the
Collier Boulevard-Green Boulevard Intersection.
• Task 1—Collier Boulevard Lane Drop South of Green: The lane-drop signage (750
FT Advance Warning) will begin approximately 300 ft south of Green Boulevard. The
lane drop (495 LF- 45 MPH) will then begin with striping and the additional pavement
will eventually transition into a U-Turn lane. The Collier Boulevard median will be

modified by 18th Ave SW to provide a northbound Collier Blvd. to westbound 18th Ave
SW direction left.
This Work Directive provides for the CONSULTANT'S services to be modified to add the
following Design Scope of Services to Collier County Project No. 68056. The following
CONSULTANT Services are an addition to the original contract:
PART 2: SCOPE OF WORK
Task 1 —Collier Boulevard Lane Drop South of Green- $27,630.00(Lump Sum)
Collier County Growth Management Division staff has requested that CH2M HILL modify the
current Collier Blvd. III - 100% Plans (April 2014) to shift the proposed southbound 3-lane to
2-lane lane drop so it occurs completely south of the Green Boulevard Intersection (Signage as
well as lane reduction). This effort will require additional plans as well as modifications to the
100% Plans, Details, Quantities and Estimates.
Services required include Tasks:
1.1 —Modify and Update the Summary of Pay Items Sheet to reflect the new and
revised quantities.
1.2 - Modify and Update the Drainage Maps to reflect the new project limits and
additional pavement.
1.3 - Modify and Update the Typical Sections to reflect the new project limits and
additional pavement.
1.4—Modify and Update the Project Layout Sheets to reflect the new project limits
and new sheet numbering
1.5—Add New Sheets and Modify and Update the existing Plan and Profile Sheets to
reflect the new project limits and additional pavement.
1.6 - Add New Sheets and Modify and Update the existing Cross Section Sheets to
reflect the new project limits and additional pavement.
1.7 - Add New Sheets and Modify and Update the existing Traffic Control Plans to
reflect the new project limits.
1.8 - Add New Sheets and Modify and Update the existing Signing and Marking Plans
and Estimates to reflect the new project limits and reconfiguration.
1.9—Update Quantities and Cost Estimates to reflect the new project limits and
reconfiguration
1.10 - Update Computation Book to reflect the new project limits, reconfiguration and
quantities
1.11 —Provide a QA/QC Review and Coordination of new and revised plans, details,
quantities and estimates. Coordinate changes with County staff and effected
departments such as landscaping.

Benefits of these modifications/improvements include:
• Improved traffic operations of the Collier Boulevard-Green Boulevard
Intersection
• Increase factor of safety for the 3-lane (proposed) to 2-lane (existing) transition
• Improved Constructability of Collier III: Phase II Project (Green to Golden Gate
Canal) with minimal throw-away costs.
• Addition of Collier Blvd. northbound Directional Left into 18`h Ave. SW
• Addition of southbound Collier Blvd. U-turn lane
PART 3: PROJECT SCHEDULE:

• Plan changes shall be incorporated into Bid Plan Submittal
PART 4: EXCLUSIONS:
The following tasks are not included in the Scope of Work for this Change Order:
