MEETING MINUTES
OCTOBER 26, 2015
10:00 AM
I. Call to Order
Mr.Johnson called the meeting to order at 10:05am. Roll call was taken and a quorum was
established.
II. Attendance
Members: Chairman Dale Johnson, Sue Chapin, Renato Fernandez (Excused), Janice Bundy,
(Vacancy)
County: Harry Sells—Project Manager
Others: Dayna Fendrick— Urban Green Studio, Wendy Warren—Juristaff.
III. Approval of Agenda
Ms. Chapin moved to approve the Agenda. Second by Ms. Bundy. Carried unanimously 3- 0.
IV. Approval of Minutes: October 7,2015
Tabled until the December 2, 2015 meeting.

VI. New Business
A. Committee Member
The Committee discussed Mr. Johnson's application for the open position for the term October 28,
2015 to October 27, 2019 noting the contributions he has made to the Committee.
• Mr. Johnson expressed his desire to be reappointed to the Committee.
Ms. Chapin moved to recommend Mr. Johnson be reappointed to the Radio Road East M.S.T.U.
Advisory Committee. Second by Ms. Bundy. Carried unanimously 3-0.

B. Median Refurbishment Bids
Mr. Sells presented the price quotes for the median refurbishment landscape projects noting:
Shell Area
The Shell Area landscape project went out to bid and Ground Zero was the sole respondent
with a price quote for the work of$4,882.
The Committee specified the variety of Croton plant approved is "Mammy". Ms. Fendrick,
Urban Green Studio,will email Staff to confirm the Committee's choice of Croton plant.
Ms. Chapin moved to approve Ground Zero Landscaping, contract E269 dated October 20,
2015, to plant the Shell Area at a price Not-To-Exceed$4,882.00. Second by Ms. Bundy.
Carried unanimously 3—0.
Amado Lawn for Asian Jasmine and Mimosa Replacement
Mr. Sells reported Amado Lawn Service, LLC submitted the "low bid"for replacement of the
Asian Jasmine and Mimosa plants in the median. Scope of work includes removal of dead
plants and up to six inches of top soil, herbicide treatment, replanting and mulching of areas.
The vendor's Collier County application documents were submitted to the Committee for
review.
The Committee discussed the proposal and cited unfamiliarity with the contractor's work. Mr.
Sells note this is not a County contract,rather a"quote"for one job. Plant material is
warrantied for one year. Protocol dictates that if the original contractor declines to perform,
the next lowest bidder is awarded the project.
Ms. Fendrick and Mr. Sells will supervise project installation and report progress to the
Committee.
Ms. Chapin moved to recommend approval of the Amado Lawn Service, LLC. contract in the
amount of$61,154.19 to the Board of County Commissioners for removal of the Asian
Jasmine and Mimosa and replanting of the medians. Second by Mr. Johnson. Carried
unanimously 3—0.
Mr. Sells will notify the Committee if the project is approved at the November 10, 2015 BCC
meeting.
The Committee requested Ms.Arnold write a letter after the November 10, 2015 BCC meeting
updating ongoing activities and commenting on the Committee's intentions.
