added:The 60% Phase of the Chokoloskee Bridge Replacement is being redesigned by the Primary Consultant TYLIN
under contract#11-5639.This redesign was approved by the BCC on 09/10/2013 by change order No. 1 (Item No.11G).As
a result of the redesign, CardnoTBE contract#09-5262CI (Bridge)was suspended on 09/10/2013.This change order seeks
to reinstate the contract by adjusting time and provides a fee increase to redo the independent peer review of the redesigned
60%plans.
2.Provide detailed rationale for the requested change: The services of CardnoTBE are needed to provide independent
peer review of the design documents and calculations because the evaluation and analysis of bridge structures are not
available in house and the County typically utilizes the services of consultant to provide these services. Since the redesign of
the 60%phase will result in a significant change in the project, a second independent peer review became necessary.
3.Provide explanation why change was not anticipated in original scope of work:The Chokoloskee Bridge
Replacement 60% Phase redesign was as a result of public feedback and was approved by the Board. These events could
not have been anticipated; therefore, no provision was made in the original scope for a second independent peer review of
the 60%design phase.
4.Describe the impact if this change is not processed: In house staff is not able to provide the level of structural analysis
and evaluation that is required for public, health, and safety. If this change order is not approved, the County would not be
providing due diligence in ensuring the public benefits.

1) SCOPE STATEMENT
This scope of services is for the independent peer review and consultation on all
design engineering drawings and specifications done by others to insure their
accuracy and to resolve issues of concern with constructability, relevant codes,
including County, FDOT and AASHTO standards, specifications, and recognized
engineering practices. The objective is to avoid major changes during construction.
As part of the independent peer review services the CONSULTANT will include but
not be limited to the independent confirmation of the following:
1. Compatibility of bridge geometry with roadway geometrics including typical
sections, horizontal alignment, and vertical alignment.
2. Compatibility of construction phasing with Traffic Control Plans.
3. Potential Conflicts with underground and overhead utilities.
4. Compliance with County, FDOT and other agency design requirements.
5. Conformity to County and FDOT Standards.
6. Structural Analysis Methodology and assumptions.
7. Completeness and accuracy of bridge and roadway plans.
8. Constructability issues.
2) PURPOSE
The purpose of this Exhibit is to describe the scope of work and the responsibilities
of the CONSULTANT and the COUNTY in connection with the preparation of
reports based on the review of the construction documents and calculations
provided during the resubmittal of the 60% design phases of the Chokoloskee
Bridge Replacement, herein called the Project.
3) SCOPE OF SERVICES
A. Project Description:
The principal intent of the Project is to replace the existing functionally obsolete
bridge (030161) located 5.4 miles south of US41 and on CR 29 (Copeland Avenue)
over the Chokoloskee Bay in Collier County, Florida. The existing bridge has been
deemed scour susceptible and was constructed in 1955. The replacement bridge
will have two lanes with bike lanes/shoulder on each side and sidewalks. The 60%
Phase was resubmitted based on feedback from the public information meeting.
The changes include: the bridge was made higher and substructures redesign.

B. Project Tasks:
This scope of work will cover the following tasks:
Task 1 R: Provide constructability review of the Project at the end of the 60%
design Phase. This includes the review of the roadway and structural
plans, design, design studies, design calculations for the items that have
changed as a result of the 60% resubmittal and verification of
incorporation of the original review comments. The constructability review
should concentrate on the ability of specific details to be constructed within
the requirements of the plans and to propose better details if possible.
Task 2R: Prepare a detailed set of constructability review comments based on the
work performed under Task 1 R.
Task 5R: Meeting and or coordination with the County and the Project designers
C. Project Design Assumptions:
All standards will be prepared in following formats:
Design Specification: AASHTO LRFD Bridge Design Specifications—4th Edition
CADD Platform: Bentley Microstation V8
Specifications: Collier County Specification Format prepared in WORD
All design calculations used for the development of standards are cataloged in the
project calculation book and will be made available to the reviewer.
D. Work Plan:
The Independent Design Peer Review for Constructability Review Check involves
the following activities:
I. Structural, constructability review of the Project
Provide complete comments on the following bridge elements as detailed in
the plans.
i. Review the structural plans and insure that they are
constructible, consistent, clear and complete.
ii. Provide check of quantity calculations and cost estimate and
provide guidance as required.
II. Provide review of the supplemental roadway plans
i. Review the roadway plans for completeness, constructability.
ii. Provide check of quantity calculations and cost estimate and
provide guidance as required.

III. Meetings/ Coordination
i. The Consultant shall attend one review meetings with County
staff and design consultant. The meeting will occur at the end
of Task 2R after the submittal of the constructability review
comment documents for the purpose of discussing the
comments.
E. Deliverables:
Consultant will furnish documents as required by the County to adequately
address the independent design peer review check comments. The
CONSULTANT shall provide the County copies of the documents for
distribution, as follows:
-Constructability (60%) review comments 2 sets 8.5" x 11" bound
4) OMPENSATION
The fee for this project will be a negotiated lump sum basis for all Tasks in
accordance with the AGREEMENT FOR PROFESSIONAL SERVICES as detailed
in Attachment 1.
