Change: Page 3, 5th bullet from top of page should read; "Observed several burnt spots

VI. LANDSCAPE MAINTENANCE REPORT
Robert Kindelan,Commercial Land Maintenance gave a brief Maintenance
Report:
• Problems with the mixing chamber pump; it did not pump 4-5 days last
week.
• Pump needs to be checked every couple of days.
He asked Roger Dick to advise CLM when alarm goes off for that location.
• Stopped herbicide treatments last month.
• Plants are dry and pulling weeds, when the ground is dry, damages branches
and vines.
Robert Kindelan and Scott Windham voiced concern for the need of water to
sustain plants and noted CLM has replacement plants to plant that will need to be
watered.
Darryl Richard stated the County received a quote from Naples Electric in the
amount of$1,169.06 to make fix the system. He noted there could be additional
costs depending on unforeseen parts or repairs that may come up. He expects to
receive the purchase order tomorrow, for Naples Electric to make repairs.

Al Benarroch,Affordable Landscaping Service&Design,LLC briefly reported:
• Trees pruned on Radio Road.
• Still need to replace one(1) Sabal Palm and one(1)Tabebula
• Dead Lantana dug out and replaced with silhouettes.
• Sprinkler system is working well.
• Asked Roger Dick to give more water.
• Continue to battle weeds.
• Irrigation system is working.
• Helen Johnson's were sprayed on median 27 &28.
IX. PROJECT MANAGER'S REPORT
Darryl Richard indicated his report items were addressed throughout the meeting. He stated
his focus will be to get purchase order for Naples Electric to get the pump repaired as quickly
as possible.
Discussion was ensued on the hedge on the Devonshire commercial side not being water. The
Berkshire common side is not interested in doing anything. MSTU will focus on maintaining
what the MSTU is responsible for.

RE: Radio Road and Devonshire Monthly Maintenance Report
MSTU Committee and Staff:
The following is a report of the general conditions and recommendations for the
maintenance of the Radio Road MSTU areas on Radio Road and Devonshire Blvd.
DEVONSHIRE:
In general the medians look healthy and well maintained with the following action items:
• The majority of the Asian Jasmine has thatched together fairly tightly with the
exception of a few bare patches. Now, allow the Asian Jasmine to flush out
between sheerings to allow overall foliage mass to get +/- 8" thick. Then
continue to top sheer and mechanically edge the Asian Jasmine along back of
gutter and edge of sidewalks on a regular basis to keep the plant masses neat
and tight. Edge Asian Jasmine off BOC in medians as well.
• CLM is continuing with a regimen of Basagran herbicide for the nut sedge and
Fulsiade for the Torpedo Grass and other grassy weeds. They are additionally
applying preemergent herbicides to combat new weed growth. They need to
continue to hand pull the larger weeds.
• CLM will prepare a price to fill in the gaps with new Asian Jasmine plants (12" on
center) in order to make up ground in the open areas.
• Continue to monitor irrigation coverage and switch out nozzles if getting better
results with conventional spray in lieu of Precision nozzles where needed.
• Trim and reduce Lantana at median tips and trim back off of curbs. Trim tall
lantana at Santa Barbara median tip to blend better with new Lantana.
• Trim Silhouette Bougainvillea at lakeside park and in medians to tighten up.
These need to be sprayed for chewing insects as well.
• It was discussed that CLM was directed by the county to stop trimming the
Viburnum hedge along the east ROW until the code violation dispute is resolved.

It was observed that the hedge was beginning to significantly encroach on the
sidewalk.
• The medians need to be weeded.
• Trim the dead Areka Palm fronds and White Bird of Paradise leaves in the north
buffer by Santa Barbara.
• It was discussed that there may be a valved direct supply line from the well pump
station to Devonshire which bypasses the mixing chamber. CLM will confirm and
report back.
• GENERAL: Trim seed pods from Montgomery Palms.
• GENERAL: The Clusia hedge needs to be topped down to 8' ht. And side
trimmed to maintain hedge form. Remove vine weeds from Clusia hedge as well.
The following replacements were noted and should be put in the rotation if and when
directed and authorized by Radio Rd MSTU committee:
• RECOMMENDATION: Remove dead or nearly dead Lantana in median 2, 75 ft
north of the lakeside park and at the Santa Barbara median tip and fill in with new
where needed.
RADIO ROAD:
In general the medians look healthy and well maintained with the following action items:
• GENERAL: Clean up and remove dead fronds from medians
• GENERAL: Weeding needed in most medians(median 16 the worst)
• GENERAL-Trim Jatropha and raise canopies.
• GENERAL: Remove seed pods from all Montgomeries and Foxtails.
• GENERAL: Let the Hamelia grow a bit to allow it to bloom. Cutting so tightly that
the flowers are never enjoyed.
• Median 1 —Replace +/-20 dead Little Red Euphorbia
The following replacements were noted and should be put in the rotation if and when
directed and authorized by Pam Lulich:
• Medians 3,4, 5, 8 (east end) 13 (east end), 18, 22, 23, 27 (west tip) previous
Juniper areas need to be replanted with Ilex or Green Island Ficus
• Median 11 Replace dead Lantana with Flax Lily at west tip
• Median 21 - Replace 3 yellow Tabebuia with Natchez Crepe Myrtle.
• Add/fill in 3 Muhly Grass east of Livingston in median 14.
• Replace one dead Sabal Palm west of Devonshire in median 28.
• In general, keep working towards replacement of all Dinnis with Dianella moving
forward as they decline

RE: RADIO ROAD M.S.T.U. Maintenance Consulting Services per the Fixed Term
Professional Landscape Architectural Services Contract.
Dear Mr. Richard:
As requested, we have prepared the following scope of services for your and the MSTU
Committee's review and approval.
This proposal includes maintenance consulting services to be provided on a monthly basis
generally coordinated with the M.S.T.U. Advisory Committee monthly meetings. In addition, the
limited refurbishment design services will be quoted on a per project basis due to the unknown
requirements of the work to be performed. These services include Radio Road (west of Santa
Barbara Boulevard and Devonshire Boulevard.
Understanding of proiect and required deliverables
We understand this maintenance consulting scope of services to be for the residents of the
Radio Road MSTU area to preserve and perpetuate the health and vigor of the existing median
and right-of-way landscaping while thoughtfully planning for its future enhancement together
with the MSTU Committee and Collier County staff. This will involve monthly site reviews,
monthly MSTU Committee meeting attendance, preparation of monthly status reports and
review of maintenance contractor report sheets. Additionally, as landscape architect design
consultant, Windham Studio will complete miscellaneous design services from time to time to
refurbish the project areas as directed and prioritized by the Committee. Lastly, we will assist
the committee and county staff in reviewing and refining the annual maintenance specifications
for the project areas and assist in the bidding process.
Project Scope
As the primary team leader, Scott Windham will personally attend all advisory committee
meetings and project site walk throughs. He will also review the General Maintenance Report
Sheets and prepare monthly written reports to inform the committee and county project
manager of project status, contractor performance and related action/resolution items. The
monthly status reports will be submitted to the county project manager a minimum of 3 days
prior to the MSTU meetings.

LANDSCAPE MAINTENANCE CONSULTING SERVICES SCOPE OF SERVICES
Windham Studio, Inc. agrees to provide the following services:
TASK 1. Attend M.S.T.U. Meetings
Windham Studio will attend all Radio Road M.S.T.0 Advisory Committee publicly advertised
meetings. (This proposal assumes one meeting per month with the committee totaling 12
meetings)
TASK 2. Field Review Meetings
Windham Studio will complete one (1) site walk through per month in the field to review the
projects. Project review will include two (2) hours to complete each site visit for a total of (2)
hours per month. Windham Studio will attend all of the monthly walk throughs.
TASK 3. Review General Maintenance Report Sheets
Windham Studio will review and comment on the "General Maintenance Report Sheets" which
are required to be submitted by the Maintenance Contractor with pay request submittals.
Windham Studio will review estimates provided by Contractor with recommendations.
TASK 4. Monthly Field Reports
Monthly Field Reports: Windham Studio will prepare monthly written and/or oral comments
and recommendations based upon on-site observations of the M.S.T.U. areas addressing:
improvements, landscape plant and irrigation maintenance problems or deficiencies. This
includes evaluation of Contractor performance per contract specifications. Windham Studio will
submit the monthly written report to Collier County Alternative Transportation staff three (3) days
prior to M.S.T.U. meeting.
OPTIONAL SERVICES:
TASK 5. Upon Request Limited Refurbishment Design Services
1. Windham Studio will provide limited (minor) refurbishment design services and observation
of installation on an as requested basis, i.e. deliverables/functions might be as follows:
a. Diagrammatic area sketch planting plans with a recommended plant list.
b. Plant field locations by staking, flagging or paint marking.
(All proposed refurbishment services will be presented to the County Project Manager for review
prior to presentation to the M.S.T.U. Advisory Committee)
TASK 6. Upon Request Miscellaneous Services (Estimated)
1. Windham Studio will provide assistance in developing the contract maintenance
specifications for purposes of bidding.
2. Windham Studio will attend pre-bid meetings and assist in preparing any necessary
addendum related to maintenance contract bidding.

TASK 7. Reimbursable Expenses
1. Includes reimbursable expenses such as courier services, prints, copies, large format
printing, etc.
It is assumed that Windham Studio will retain the ability to request that the Project Manager shift
funds and/or re-distribute activities between tasks, if tasks require different levels of effort than
originally estimated.
FEES: Please see attached Fee Schedule
Maintenance Consulting Services fees will be billed monthly on a time and materials basis per
the attached fee schedule.

Municipal Service Taxing Unit(MSTU)ADVISORY COMMITTEE
BY LAWS
ARTICLE I
Name
The name of this Advisory body shall be the Radio Road Beautification Municipal Service
Taxing Unit(MSTU)Advisory Committee
ARTICLE II
Jurisdiction
The Advisory Committee shall function as an advisory body to the Board of County
Commissioners (BCC) and is governed under Ordinance 96-84, as amended. The Committee
will make recommendations to the BCC in matters related to Beautification projects within
the district, funding, preparation of bid documents, management of contracts, and
managing the annual budget. The BCC shall have the final authority over the actions of the
MSTU.
ARTICLE III
Officers and their Duties
Section 1. The officers shall consist of a Chair and Vice-Chair.
Section 2. The Chair shall preside at the meetings and shall have the duties normally
conferred by parliamentary usage of such officers.
Section 3. The Chair may call special meetings, appoint s ubcommittees, and act as a
liaison with government bodies and other related committees.
Section 4. The Chair shall have the privilege of discussing all matters before the
Committee and shall have the same voting rights as all Committee members.
Section 5. The Vice-Chair during the absence of the Chair or the Chair's inability to
serve, exercise all of the duties and powers of the Chair.
Section 6. The full Committee may participate in the election process and vote.
Section 7. The Advisory Committee shall adopt procedures by which it shall operate.

ARTICLE VI
Election of Officers
Sections 1. Nomination of Chair and Vice-Chair shall be made from the floor at the
annual organization meeting in February of each year, and the election shall be held immediately
thereafter.
Section 2. A candidate receiving the majority vote shall be declared elected and shall
serve a term of one (1) year, or until a successor shall take office. An Officer is eligibility for
reelection if the term of the Advisory Committee is extended.
Section 3. Vacancies in the position of Chair and Vice-Chair shall be filled
immediately by regular elections procedures.
Section 4. The Chair or Vice-Chair may be removed by a super majority of the
Committee with or without cause.
ARTICLE V
Committee Structure and Meeting Requirements
Section 1. The Radio Road Beautification Municipal Service Taxing Unit (MSTU)
Advisory Committee shall consist of five (5)members.
Section 2. Committee Appointments - Appointment of members to the Advisory
Committee shall be by resolution of the BCC, which resolution shall set forth a date of
appointment. Each appointment shall be for a term of four years. Appointments to fill any
vacancies on the Advisory Board shall be for the remainder of the unexpired term.
Section 3. Regular Meetings — Regular meetings of the Advisory Committee shall be
held on the second Monday monthly at 3:00 p.m., Berkshire Lakes Master Association Clubhouse
located at 495 Belville Boulevard, Naples, Florida 34104. If a regular meeting is required to be
held at a time and location other than what is specified herein, notice will be provided at least 72
hours prior to the meeting and posted on the County website.
Section 4. Special Meetings — Special meetings of the Advisory Committee may be
convened by the Chair upon giving notice thereof.
Section 5. Notices —Notices of an Advisory Committee meeting shall be given to all
members and to the Public at least seventy-two (72) hours in advance of the meeting. At any
meeting, the Committee may set a future meeting date. Staff may provide notices and agendas
to members at least seventy-two (72)hours prior to meetings.
Section 6. Attendance—Members may notify the Staff Liaison or the Transcriptionist if

they cannot attend a meeting. If a member misses two (2) successive Committee meetings without
a satisfactory excuse, he/she may forfeit his/her appointment.
Attendance is preferred in person but may occur telephonically provided that (1) there is a quorum
of the Committee members physically present; and (2)the members who are present both authorize
the participation by telephone, and find that the absence was due to "extraordinary circumstances."
Once approved to participate, the absent member can conduct themselves in the same manner as if
they were present, including making motions and voting on items.
Section 7. Quorum —A quorum of the Advisory Committee shall consist of three (3)
members and an affirmative vote of a majority of those present and voting shall be necessary to
pass any motion. In the absence of a quorum, those members present may review the agenda
items and make recommendations that may be reviewed for approval by the Advisory Committee
at it s next meeting where there is a quorum present.
Section 8. Voting — Voting shall be by voice vote, or show of hands, if necessary, and
may be recorded by individual (or group).
Section 9. Meetings - All meetings shall be open to the public to encourage public
awareness and participation.
Section 10. Minutes -Minutes shall be taken at all Advisory Committee meetings. Voice
recording of meetings is acceptable. A written record of meetings, resolutions, findings and
determinations shall be kept for the Advisory Committee.
Section 11. Documents - The Alternative Transportation Modes staff shall assist with
preparation, duplication, and distribution of materials necessary for meetings. Copies of all
Advisory Committee minutes, resolutions, reports and exhibits shall be submitted to the
Clerk's Office and posted on the County Website for public viewing.
Section 12. Except as otherwise provided in these Bylaws, Roberts Rules of Order shall
be followed at all meetings.
Section 13. Agendas — Agendas are set by the Chair with coordination with the Staff
Liaison and Transcriptionist. Members may send agenda items request to the Staff Liaison and/or
Transcriptionist prior to the publishing of the Agenda. Otherwise,the agenda may be changed at the
meeting prior to approval of the Agenda.

ARTICLE VII
Public Participation
All Radio Road Beautification Municipal Service Taxing Unit (MSTU) Advisory Committee
meetings will be public meetings as required by Florida Statutes. Participation by the Public is
encouraged and in an effort to be fair to all wishing to participate, the following rules shall apply
when receiving public comment.
Section 1. Public Comment on Items on the Agenda-During each portion of the agenda
the Chair may open up the meeting for public comments.
a. The Chair shall declare when the floor will be open to public comments.
b. The Chair shall declare when the public comments period for the Agenda item is closed
for public comments. Once the item is closed for public comment there shall be no
further comment from the floor.
c. All persons wishing to speak on an Agenda item must register with the Staff liaison prior
to the presentation of the Agenda item by completing a speaker's slip.
d. Each public participant must identify themselves by name and address.
e. All registered speakers will receive up to three (3) minutes to speak unless the time is
adjusted be the Chair.
f. There will no sharing of minutes among those present in an effort to extend the time
permitted to speak.
g. Once a participant has been afforded the opportunity to speak he/she may not speak again
unless requested by the Chair.
Sections 2. Public Comment on Items NOT on the Agenda—A Public Comments section
will be included in each agenda. During this portion of the meeting items that have not been
brought forward previously or were not on the agenda may be discussed or raised by the Public.
a. All persons wishing to speak on under public comments on items not on the Agenda must

register with the Staff liaison prior to this portion of the Agenda b y completing a
speaker's slip.
b. Each public participant must identify themselves by name, address, and specify subject
matter.
c. All registered speakers will receive up to three (3) minutes to speak unless the time is

ARTICLE VIII
Amendment to By-Laws
Section 1. Any proposed permanent amendment to the By-Laws shall be provided
to every Advisory Committee mem ber at least seven seventy-two (72) hours before it is voted on
at a regular meeting.
Section 2. These Bylaws, and any and all amendments to the By-Laws, will become
effective upon an affirmative vote of a majority of the quorum.
