The Meeting was called to order by Chairman Dale Johnson at 3:30 P.M.
Roll call was taken and a quorum was established.

Darryl Richard distributed the Radio Road East MSTU — Fund 166
Report date October23, 2012. (See attached)

Staff reported not receiving a response on how the uncollected Ad
Valorem taxes will be recorded in the budget in the new fiscal year.

Staff noted a copier had to be leased to accommodate each of the
MSTUs needs. Each MSTU will be charged 1-month of the yearly
lease plus copy charges.
B. Project Manager Report
Darryl Richard distributed and reviewed Change Order #1 (12-5878)
Proposal on the Landscaping & Irrigation Project from Hannula
Landscaping and Irrigation, revision date October 18, 2012. (See
attached) Cost factors were updated due to field information needed to
be verified on the actual quantity of the HDPE pipe and the field
installation in the amount of$12,791.
Change Order #1 was voted on and approved for $13,069.50 at the
September 18, 2012 meeting.
He reported AgTronics shipped supplies under the P.O. approved
by Advisory Committee but when the Finance Department did not pay
AgTronics invoice for non-bid items; AgTronics picked up shipment
the same day and returned it to inventory.
Darryl Richard explained another option for the Advisory Committee
to take would be to accomplish purchasing would be by piggy backing
off Hannula contract. This would include paying tax and contractor
product mark up of 10%.
Staff distributed and reviewed Change Order #2 (ITB12-5878) in the
amount of$23,661.70 dated October 18, 2012. (See attached)
Dale Johnson asked how much more it would cost because of these
procedural changes and who didn't do what they were supposed to do.
Discussion took place on the RFP process, modifications made on bid
items and the cost difference due to procedural changes, error(s) and
miscommunications. The Advisory Board opt not to hold up the
project; however indicated a letter be addressed to the BCC explaining
what happened due to internal rules and procedures.
Dale Johnson moved to accept Change Order (#2) as presented;
however and that in addition the (Staff) attach a letter, signed by the
Advisory Board, to the Change Order outlining the issues associated
with that change to make BCC aware of what happened not to exceed
$36,452.70. Second (under protest) by Sue Chapin. Motion carried
unanimously 3-1. Tom Depouw opposed.
It was noted a letter attached to the Change Order in the Consent
Agenda may not be noticed by the Commissioners.
A consensus was formed to send the letter directly to each County
Commissioner and forego the letter during the approval process.

VI. Landscape Architect Consultant Report
A. Landscape Design
Dayna Fendrick distributed a Radio Road East MSTU Landscape
Architects Field Report # 5 and #6. (See attached)
She reviewed the following:
D Hannula is ready for the laterals. In process of killing sod over
the next 5-10 days.
➢ FPL is scheduled to install power on October 26.
D Rocks found during the process will be hauled away.
D Staking for tree location has been completed.
➢ Planting should start approximately November 1.
Urban Green reported having a hard time finding Silver Button Woods
and may need to go with a smaller size.
Dayna Fendrick addressed many questions posed by Advisory Board.
Dale Hannula provided and reviewed the Landscape and Irrigation
Project Schedule dated October 18, 2012. (See attached)
Discussion was made regarding the traffic light at Madison Park and
the current issues including developers setting aside the funds for the
traffic light installation. Conflicting information has been received
regarding traffic light installation.
Darryl Richard will submit contact information to Dale Johnson.
VII. New Business
A. Vote on Member Applications
Darryl Richard presented Advisory Board Applications from Tom
Depouw and Sue Chapin. He noted the positions had been advertised
publicly.
Dale Johnson moved to accept applications from Tom Depouw and
Sue Chapin and recommend approval by the BCC. Second by Renato
Fernandez. Motion carried unanimously 4-0.
VIII. Old Business
A. Liaison Report—Radio Road MSTU—None
B. Status on RFP for Architect Consultant
Darryl Richard reported a representative from the Horticulture Extension
Office has agreed to join the Selection Committee. The Selection Committee
will consist of 5-Advisory Board Members, Darryl Richard and Extension
Representative. Staff will submit a letter to the BCC for their final approval.

Subject: Motorola Material Supply for Radio Road East MSTU Project
Dear Commissioners:
On January 31, 2012, 73%of the voters approved a ballot question for a loan to advance the construction of the beautification project
on Radio Road East of Santa Barbara Boulevard to Davis Boulevard. As a result of this overwhelming support, the Radio Road East
MSTU Advisory Board is now taking the necessary steps to complete the project as quickly as possible. The project is now underway
with expected completion in January 2013!
At the October 23,2012 Radio Road East MSTU regular monthly meeting, staff informed us that the purchase order previously issued
for the purchase of Motorola material supply for the project had to be cancelled in that the total for non-bid material was over the
purchasing threshold of$3,000 for these type of items. It was explained during the meeting that the items were not included in the
tabulation for the Ag-Tronix annual contract#10-5520 Motorola Central Irrigation Supplies& Service. It appears that the technology
for the typical Motorola Controller has changed since the Contract#10-5520 was approved by the Board of County Commissioners on
May 10,2011. And as a result some items required for our irrigation controller are not included in the current bid.
The purchase of these non-bid items has to now be placed into our contractor's Contract 12-5878 with Hannula Landscape &
Irrigation so that the contractor can complete the installation per contract. We now have to pay$500 for Florida 6%sales tax,another
$500 for bid bond increase, and a 10%mark-up for overhead and profit. In total the Radio Road East MSTU will have to pay about
$12,000 for what did cost approximately$9,000 due to these contractual increases.
We as an advisory committee to the Board of County Commissioners would like to bring this to your attention so that there may be
some amendment to the existing contract for Motorola material supply in coordination with the County Purchasing Department,
Clerks Finance, and other County staff members as appropriate to ensure that the Motorola Contract serves the purpose as it's
intended for Motorola material supply for county projects. Please coordinate this request through our District 3 Commissioner's
Office and our duly appointed coordinator Sue Chapin advisory committee member so that we can track the progress towards
resolving this issue with the purchase of Motorola products for county projects.

NOTES/OBSERVATIONS
1. Darryl provided copies of the processed Change Order#2 and Work Directive for Hannula LS,which includes the Motorola
Controller and the work already performed on additional directional bores. The C.O.still has to be approved by the BCC, but
Purchasing has indicated to Darryl that Hannula can move forward with the work and invoice for the controller.
2. Darryl called Sonya with Agtronix to see if they would be willing to release the controller,and Sonya indicated they would.
Dale is going to coordinate delivery of the controller to his electrician with Sonya.
3 Dale stated that the lateral line installation will be complete today,and that he is at a standstill until we get electric power.
He needs to have power in order to start up the pump and flush the mainlines,which must be done before the control valves
and heads are installed.
4. Darryl had received an email from Miguel with FPL that they were to be doing the power drop today; after the meeting we
saw the FPL crews were on site working on the pole.
5. Dale mentioned that there is one sleeve at the eastern limit of work that needs to be extended; the paver crew had
excavated for the paver area and found the sleeve is in place but needs to be extended beyond the pavers. The sleeve may be
8"instead of the typical 6".
6. The paver crew will be excavating and pouring the concrete base this week. Paver installation will start next week.
7. The tilling of the soil is complete; they did encounter quite a bit of rock,and will remove the rock from the medians.
8. Hannula will start bringing in the additional topsoil and getting contours established next week. They will white line the
bedline/shell areas and flag tree locations to help determine the contour layout.
9. Dale expressed a concern with the trees that have been tagged,and that the nurseries may not hold them much longer.
He would like to invoice for stored goods so he can pay a deposit to the growers to hold the trees. Dale will invoice for stored
goods on the November invoice.
10. Shelly w/Hannula had provided paver samples and a crushed shell sample at the meeting on 11/14/12. Darryl wants to
get all physical samples in hand,with approved labels on them for the record.
11. After the meeting,we all met at a median on the Phase 3 portion of Radio Road to compare the paver samples with the
pavers that were installed on Phase 3. The paver colors were matching,and were approved by Darryl and Dayna.
12. Dayna has the crushed shell sample,and will return it to Darryl to hold. The topsoil sample is at Hannula's facility,and
Darryl will pick it up.
13. The water main locates were done on 11/6/12 by Steve Chandler with Stakes&Locates. There are two areas where we
have potential conflicts-one on the second median east from Santa Barbara Blvd(#32),and one at the median near
Sherwood Forest(#37).
Based on measurements taken from those flags to the tree locations and offsets from the water main and curb, on
median#32,it appears that the group of 4 Royal Palms will need to be substituted with a smaller palm.There is not
sufficient room to maintain a 5'offset from the water main and 10'from the travel lane. Dayna suggested using Buccaneer
Palms as the substitute,but using two Buccaneer in place of one Royal since they are smaller scale.

On median#37,it appears that there may be sufficient room to shift the Royal Palms to the north and still maintain an
adequate distance from the travel lane. There are also a couple of Gumbo Limbo trees that may need to be shifted,but it
looks like we have sufficient offset from the travel lane.
All tree locations will be reviewed again once the locates are refreshed;the flags were disrupted by the tilling of soil. We
will request that both Utility Dept staff and ROW Section attend a field visit to review the adjustments.
14. Tree planting should be started in the next two weeks,once the grading is finished.

Proposed Irrigation System Construction — Initial Installation Memorandum

PROJECT: RADIO ROAD EAST (Phase IV)—Naples, Florida
SUBJECT: Irrigation System Construction Initial Activities.
Although the weather has been favorable during the past three weeks the system's installation has been slow
due to not having electric service that would have enabled the Irrigation Contractor to proceed at a faster pace.
The following is list of the activities that have been observed during the past three weeks.
ELECTRIC SERVICE
The electric service meter and other service hardware were installed on Wednesday November 28, 2012. The
electric service wire from the meter to the pump station needs to be installed. The electric service line (wire) is
going to be installed five feet from the back of curb at the median's south side. Governing laws/codes/rules &
regulations specify for a minimum depth of not less than 24". The electric service wire is going to be encased
by a SCH 80 PVC (gray). Once that the electric service is installed the connection,testing and initial activation
need to take place. This course of action needs to take place ASAP (November 30, 2012) so that the Irrigation
Contractor can proceed with the pump station's initial activation, main line flushing and pressure testing.

IRRIGATION PUMP STATION INSTALLATION
Pump Station Installation: The Irrigation Contractor,Owner's representative and the pump station
manufacturer need to coordinate and proceed with the initial start-up and calibration once that the electrical
service is connected and in operation. Status: Proceed with the initial start-up and calibration adjustments once
that the electrical service is connected and in operation.
IRRIGATION CONTROLLERS INSTALLATION
AC Controller(Controller C3): The budget to purchase and have the controller installed was recently
approved.Now that the electrical service is going to be available within the upcoming days the Irrigation
Contractor needs to have the concrete base form poured and formed so that the controller can be installed soon.
This controller is the only one that will have a rain sensor. The controller will provide the rain interruption
operation cease signal for the remaining controllers. The installation of the rain sensor will be done towards the
end of the system completion. Status: Proceed with the installation of the concrete base, installation of the
controller and initial start-up and calibration adjustments once that the electrical service is connected and in
operation.
DC Controllers(Controllers Cl,C2 & C4):Now that the ground is tilled and the pump station is soon to be
activated it is a good time for the Irrigation Contractor to have the concrete bases form poured and formed so
that the controllers can be installed soon. These controllers will not have a rain sensor.The rain interruption
operation cease signal will come from the DC controller(Controller C3).At a different project site recently

there have been operating and communication issues with a similar DC controller.Please coordinate with Roger
D. &AGTRONIX for the latest development of the malfunction&communication problem for the DC
controller prior to proceeding with the installation of the units.If the controller concrete base for the DC
controllers is similar/compatible with AC controller cabinets it is advisable to proceed with the pouring/forming
and installation of the bases for the referenced controllers. Status: Research the status of the conflict for the
previously referenced controller, and proceed with the installation of the concrete bases and installation of the
controllers if the conflict(s)have been resolved favorably.
CONTROL WIRE INSTALLATION
The control wire installation initial phase is complete. The control wires observed being installed conformed to
the specifications:AWG solid copper insulated wire suitable for direct burial applications; 12 Gauge white for
ground(common)wire; 12 Gauge white w/color strip for spare ground(common)wire; One 12 Gauge spare
ground(common)wire for every ground(common)wire at the DC controllers; 14 Gauge red for activation
wire; 14 Gauge blue& green for spares.
PVC IRRIGATION MAIN LINE INSTALLATION
Main Line Piping System Installation,The installation of the 3" irrigation main line is complete. The material
included: 3"Class 200 PVC gasket pipe,Pantone Purple 522C,with Ductile Iron Pipe fittings manufactured by
HARCO. Gasket joint lubricant was used for insertion ease at all gasket pipe joints.All insert pipe sections
were marked with a black marker 4"+/-from the end to assure and provide a visual adequate insertion depth
mark. Class 150 HDPE Pipe with mechanical joint couplings with transitional gaskets(for PVC to HDPE
junction connections)were used at paved surface crossings. The average main line depth was 42"+/-. Metallic
tracing tape was installed about 9"to 12"over the main line as it was being backfilled. Electronic
locating/tracing balls were placed at each end of the main line at paved surface crossing locations, and also at
locations where the main line made directional changes of 45 degrees or more. Corrective Action Required:
Pending on flushing and initial hydrostatic pressure testing result(s).
IRRIGATION MAIN LINE FLUSHING AND PRESSURE TESTING
Main Line Flushing&Pressure Testing,The next step in the process is to thoroughly flush the main line
once that the electric service is activated and the pump station goes through the initial activation.After the main
line is flushed the air vacuum relief and pressure relief valves need to be installed. The main line specialty
valves must be installed prior to doing pressure testing.,Pending Action: Flush the irrigation main line and
pressure test it.
EXISTING SLEEVES LOCATION,EXTENSION& DIRECTIONAL BORES INSTALLATION
The locating of existing sleeves and installation of the new directional bores to be used in this phase of the
project is complete.There is a need to provide the extensions for existing sleeves 24 (6") &25 (2")because the
west end of the sleeves is currently located directly underneath(station 322+23). The sleeves will enable for the
main line and control wires to extend into the last median that extends into the Radio Road&Davis Boulevard
intersection.The sleeves&extension work are identified in the plan set on sheet LIl 10. Pending Action:
Proceed and finish the attachment of the sleeves extension.

IRRIGATION MAIN LINE ISOLATION& SPECIALTY VALVES INSTALLATION
Isolation Valves: The irrigation main line isolation valves continue being at 80%installed. The installers have
not been able to proceed with finalizing the installation of the valves due to the ground being tilled;the valve
boxes would be damaged and unsettled. The ground around the valves is now tilled. The installers can proceed
with making the access sleeve extension height adjustments and installing the valve boxes.
Rejected Isolation Valve: Through other reliable sources (other Irrigation Specialists,not the manufacturer)I
learned that the rejected valve's handle can be removed and replaced with a square nut handle (for opening and
closing the valve).Experienced staff at HLI confirmed it with me and also informed me that HLI staff does
have experience at making similar handle replacements. If they are able to replace the handle, have the valve
working properly and offer the same warranty as with other isolation valves for the system acceptance of the
valve installation and modifications may be granted as long as the Owner approves it. Pending Action:
Proceed with the finalizing the installation of the valves not replaced,and proceed with the modification of the
rejected valve if the Owner approves the modification.
Pressure Relief Valves: Not installed yet,must use SCH 80 PVC nipples and interconnecting fittings. Please
refer to the installation detail. Omit isolation valve shown attached to the interconnecting nipple. The valves
must be installed prior to doing the main line pressure testing because it is an element that is attached directly to
the main line. Status:Not installed yet,proceed with the installation prior to the main line pressure testing.
Air Vacuum Relief Valves:Not installed yet,must use SCH 80 PVC nipples and interconnecting fittings.
Please refer to the installation detail. Omit isolation valve shown attached to the interconnecting nipple. The
valves must be installed prior to doing the main line pressure testing because it is an element that is attached
directly to the main line. Status:Not installed yet,proceed with the installation prior to the main line pressure
testing.
Double Valve Main Line Blow-Off Assemblies: The blow-off assemblies are partially installed(about 75%
complete). The valve boxes need to be provided at both valves, and the nipple for the smaller valve needs to be
shortened;that will be done as the valve boxes are installed. Between the blow-off assembly large valve
(flanged connection valve)and the smaller discharge valve(threaded connection valve) a PVC pipe section
appears to be placed to mark the location of a stub-out for a main line item;most likely a main line pressure
check unit.All main line pressure check devices are to be installed upstream of main line blow-off assemblies.
The plan and corresponding detail clearly show it. At the location where it appears that the Irrigation Contractor
might have considered for installing the pressure check device the main line section will not be under pressure
because the blow-off assembly large valve is to be closed for better pressure surge protection for the threaded
connection valve at the end of the blow-off assembly unit.Do NOT install pressure check devices/assemblies
between the blow-off assemblies valves. Status:No changes.Please refer to the installation detail and use
materials that conform with the corresponding specification ... SCH 80 PVC piping and interconnecting
fittings, SLOW setting solvent weld glue.

IRRIGATION LATERAL LINES PIPING INSTALLATION
The installation of the lateral lines piping along the north side of the medians is complete. The lateral lines
piping that were observed being installed correspond to the spray&rotor head zones,plus the bubbler zones 1-
1/4" lateral line piping. The installation of the lateral lines along the south side of the median and
• interconnecting crossing lateral lines is about 85%complete at the time of preparing this report. The installation
of all lateral lines piping should be complete by the end of the day tomorrow(Friday November 30, 2012).
The installation observed so far conforms to the specifications and continues being satisfactory. Class 200 PVC
solvent weld Pantone Purple 522C pipe is being used. The solvent welding technique observed conforms to
specifications: The joint sections are primer with a primer before the solvent weld glue is applied; Solvent weld
compound is applied to both section;the sections are joined through insertion;A quarter of a turn is done to the
sections to provide an even spread of the compound;Excessive solvent weld compound that may come out of
the recently joined section is immediately wiped. The average trenching depth was 24"+/-for trees&palms
bubbler lateral lines; 18"+/-to 15"+/-for spray&rotor head hydrant lateral lines. The backfill material
observed being used was clean;the rocks and other small debris were removed.The lateral line piping
interconnection fittings conform to the corresponding specifications, SCH 40 PVC.Rigid plastic male threaded
X male barbed insert sittings continue being used for joining the flexible tubing sections to the lateral line
piping and proposed hydrants. The lateral lines for the South side of the medians will be installed after the
ground tilling is completed so that they are not damaged during the tilling operation. While waiting for the
electric service to be set up and activated for the pump station operation the installers have worked with
diligence after the ground was plowed in order to have the south side's lateral lines piping installed.

SUMMARY
The installers continue working with diligence to make progress with the project installation.Now that the
electric service is going through the process of being hooked up the installation of the remaining elements
should proceed rapidly.
