Minutes
January 8, 2013
I. Call to Order
Mr. Jones called the meeting to order at 10:02am.
A quorum was established
II. Attendance
Members: Robert Jones, George Fogg, Dick Barry, Kevin McKyton, Kenneth Bloom

III. Approval of Agenda
Mr. McKyton moved to approve the Agenda subject to deletion of item VI-4 - Phases 1 -3
Maintenance Specifications. Second by Mr. Fogg. Carried unanimously 4 - 0.
IV. Approval of Minutes - December 4, 2012
Mr. Fogg moved to approve the minutes of the December 4, 2012 meeting subject to the
following changes:
• Page 2, paragraph 1, bullet point 4 to read - "Reserves to be used for Debt Service."
• Page 2, paragraph 5, line 24 to read "Repairing the Turtle Lake's Condominium
Association irrigation and plantings."
• Page 2, paragraph 5, line 28 - delete "Estimate of general costs"
• Page 3, paragraph 1, line 2 to read - "Turn Lane including planting and irrigation.
• Page 3, line 18, delete - "George Fogg noted the $692, 000 costs was stated
incorrectly. "
• Page 3, paragraph to read - "Dick Barry motioned to have the contractors to re-
submit bids for just the sidewalk construction with lighting and repair /replacement
of grass and irrigation damaged by the construction. Second by Kenneth Bloom."
• Page 3, paragraph 4, line 3 - add reference to WSI (Windham Studio, Inc.).
• Page 3, paragraph 8 to read "George Fogg moved to add Woodshire landscaping
from Pine Ridge Road to Forest Lakes Blvd as an alternate to the original
motion."
• Page 3, paragraph 9 to read "Dick Barry agreed to amend motion to include
Woodshire landscaping from Pine Ridge Road to Forest Lakes Blvd as an alternate
to the original motion. "Second by Kenneth Bloom. Motion carried unanimously S

• Page 4, paragraph 5 to read "It was requested by Staff to investigate the sight lines at
the entrance to Forest Lakes from Pine Ridge Road and the Golf Course
Clubhouse."
Second by Mr. McKyton. Carried unanimously 4 — 0.
V. Transportation Services Report
A. Budget Report — Caroline Soto
Mr. Richard provided the document "Forest Lakes MSTU Fund 159 — January 08,
2013 " for information purposes. He noted:
• Line item 37 "Reserves" totaling $1.166M we will be utilized for debt service
payments.
• There is a light pole in need of replacement and for cost efficiency purposes, it
will be ordered at the time other light posts are ordered for the proposed scope of
work at the intersection of Forest Lakes Blvd. and Pine Ridge Road.
The Committee requested Staff to:
1. Undertake the necessary measures to replace or repair the road reflectors in
any areas deemed necessary.
2. Obtain price quotes for pavement striping of roadways for any areas deemed
necessary.
B. Project Manager Report — Darryl Richard
Mr. Richard provided a written Status Report from Scott Windham dated January 8,
2013, Re: Forest Lakes MSTU Bond Project F -58 Sidewalk Project Improvements
Status Report' for information purposes. He noted a sketch titled " "Forest Lakes Blvd
Cart Path Option" dated 12/31/12 was attached to the Report. He sought Committee
direction on the concept proposed in the sketch.
Mr. Fogg moved to direct Staff to contact the consultant and report the Committee
endorses the concept as shown on the above referenced sketch. Second by Mr.
McKyton. Carried unanimously 4 — 0.
VI. Landscape Architect Report — Windham Studio

VII. Old Business
Mr. Richard reported he researched the status of approximately $30K due to the MSTU
from the Golf Course in relation to their renovation project and determined no funds are due
as a County Staff member conducting the "plan review" misinterpreted the scope of the
project.

IX. Public Comment
Mary Jane Peck addressed the Committee and reported there are some street lights that are
not functioning as well as sign and front entrance lights.
In addition, questioned who was responsible for maintenance of the drainage Swale opposite
of the maintenance area.
Mr. Richard reported Staff was aware of the street light issue and is addressing it.
The Committee reported a contractor recently disrupted the power service for the other lights
in question.
Mr. Richard reported the Golf Course has always been responsible for maintenance of the
swales in question, and that the MSTU only performed mowing as relates to stormwater
management provided that vegetation growth inhibits the flow of water to such a degree that
mowing is warranted. And that currently inspection indicates that mowing is not warranted.
Lynne Bahula addressed the Committee requesting consideration be given to addressing
vehicular speeding within the community.
There being no further business to come before the Committee, the meeting was
adjourned by the Chair at 11:05 A.M.
